Operative Therapie bei Frauen mit invasivem Zervixkarzinom

Zusammenfassung

Das Spektrum möglicher Operationen für Patientinnen mit einem primären Zervixkarzinom reicht von der Konisation (± Sentinellymphonodektomie) bis zur primären Exenteration. Neben dem FIGO-Stadium erlangt deshalb eine exakte Stadieneinteilung essenzielle Bedeutung, um eine individuelle Therapie festlegen zu können. Jungen Patientinnen mit Tumoren kleiner 2 cm Durchmesser und histologisch tumorfreien Lymphknoten kann mit hoher onkologischer Sicherheit eine fertilitätserhaltende Operation in Form der radikalen Trachelektomie angeboten werden. Nur das laparoskopische (extra- oder transperitoneale) Staging ermöglicht bei lokal operablen Tumoren eine Differenzierung zwischen Patientinnen, die primär radiochemotherapiert oder solchen, die mittels radikaler Hysterektomie behandelt werden sollten, um die signifikant höhere Toxizität einer trimodalen Therapie zu vermeiden. Die verschiedenen Verfahren der radikalen Hysterektomie wie abdominale nervensparende radikale Hysterektomie, totale mesometriale Resektion (TMMR), die vaginal assistierte laparoskopisch radikale Hysterektomie (VALRH), totale laparoskopische radikale Hysterektomie (TLRH) oder roboterassistierte radikale Hysterektomie (RRH) zeigen onkologisch adäquate Ergebnisse, wobei vergleichende randomisierte Studien nicht existieren. Im FIGO-Stadium IVA sollte eine Patientin über die Möglichkeit einer primären Exenteration aufgeklärt werden, auch wenn keine Daten im Vergleich zur primären Radiochemotherapie verfügbar sind. Eine enge, möglichst prätherapeutische, interdisziplinäre Zusammenarbeit mit der Radioonkologie, Pathologie und Hämatoonkologie im Rahmen gynäkologisch-onkologischer Zentren ist unabdingbare Voraussetzung für eine optimale Behandlung von Patientinnen mit der Diagnose Zervixkarzinom.
