Chirurgie bei retroperitonealen Weichgewebesarkomen und gastrointestinalen Stromatumoren

Zusammenfassung

Hintergrund

Sowohl retroperitoneale Sarkome (RPS) als auch gastrointestinale Stromatumoren (GIST) sind seltene Tumorentitäten. Ihre chirurgischen Therapieprinzipien unterscheiden sich signifikant von denen anderer Tumorentitäten.

Ziel

Diese Übersichtarbeit soll eine systematische Darstellung der chirurgischen Therapieprinzipien von RPS und GIST geben.

Material und Methoden

In PubMed wurde eine selektive Literaturrecherche nach Arbeiten, die sich mit der chirurgischen Therapie von RPS und GIST befassen, durchgeführt. Es bestanden keine Restriktionen hinsichtlich Datum oder Sprache der Publikationen.

Ergebnisse

Sowohl bei RPS als auch bei GIST sind die prätherapeutische histologische Sicherung und adäquate Bildgebung essenziell für die Therapieplanung. Für beide Entitäten ist bei nichtmetastasierter Erkrankung die chirurgische Resektion Hauptsäule der Therapie. Bei retroperitonealen Sarkomen ermöglicht die Kompartmentresektion mit Entfernung aller tumordeckenden Organe die beste lokale Kontrolle und scheint das Überleben zu verlängern. In einem großen Patientenkollektiv lag die Fünfjahresüberlebensrate nach Kompartmentresektion bei 61 %, verglichen mit 48 % nach marginaler Resektion bei akzeptabler perioperativer Morbidität. Bei GIST ist aufgrund der Seltenheit lymphogener Metastasen keine Lymphadenektomie erforderlich. Eine Resektion sicher im Gesunden reicht aus, sodass häufig ein Organerhalt möglich ist. Zur präoperativen Tumorverkleinerung, welche diesen erleichtert, hat sich mittlerweile eine 6- bis 9-monatige neoadjuvante Therapie mit Tyrosinkinaseinhibitoren etabliert und wird durch Behandlungsleitlinien international empfohlen. Eine retrospektive EORTC-Studie zeigte, dass bei neoadjuvant therapierten Patienten mit fortgeschrittenen Tumoren 7 Jahre nach Resektion noch 50 % der Patienten erkrankungsfrei leben. Bei RPS kommen eine präoperative Strahlentherapie oder eine (hypertherme) Chemotherapie in Betracht. Für die präoperative hypertherme Chemotherapie zeigte eine multizentrische randomisierte Studie ein signifikant besseres lokalrezidivfreies Überleben (56 % vs. 45 %, p = 0,04) und erkrankungsfreies Überleben (34 % vs. 27 %, p = 0,04) nach 5 Jahren verglichen mit Chemotherapie ohne Hyperthermie. Bei GIST kann ein chirurgisches Vorgehen in der metastasierten Situation nur dann eine sinnvolle Option darstellen, wenn eine entsprechend Patientenselektion durchgeführt wird. Insbesondere die Resektion unter medikamentöser Therapie regredienter Tumorläsionen scheint zu einem Überlebensvorteil zu führen, prospektive Daten hierzu liegen jedoch nicht vor.

Diskussion

Die chirurgische Therapie von RPS und GIST basiert auf eigenen Prinzipien, deren Kenntnis für jeden an der Behandlung Beteiligten unabdingbar ist. Die optimale Behandlung dieser komplexen Krankheitsbilder erfordert einen interdisziplinären Ansatz. Operationszeitpunkt und -taktik müssen immer im Kontext der Möglichkeiten einer multimodalen Therapie festgelegt werden.
