Grundlagen der Kachexie bei Tumorpatienten

Zusammenfassung

Hintergrund

Das Tumorkachexiesyndrom ist das häufigste paraneoplastische Syndrom und betrifft etwa die Hälfte der Patienten mit einer malignen Tumorerkrankung.

Ziel

Der Beitrag stellt die pathophysiologischen Prozesse in der Tumorkachexie und deren klinische Folgen dar.

Material und Methode

Selektive Literaturrecherche in Pubmed mit Einbeziehung der aktuellen Leitlinien.

Ergebnisse

Charakteristisch für die Tumorkachexie ist ein systemisches Inflammationssyndrom. Tumorspezifische und entzündungsfördernde Mediatoren führen zu Appetitverlust, systemischen Entzündungsprozessen, metabolischen und hormonellen Veränderungen. Das systemische Inflammationssyndrom besitzt Auswirkungen auf den Protein-, Kohlenhydrat- und Lipidstoffwechsel der Leber sowie der peripheren Organe. Häufig lassen sich eine Insulinresistenz und eine eingeschränkte Glukosetoleranz nachweisen. Folgen sind eine verminderte Nahrungszufuhr, eine schlechtere Nährstoffverwertung und ein Verlust an Muskulatur, mit oder ohne Verlust von Fettmasse. Zusätzlich können Nebenwirkungen der onkologischen Therapie den Muskelabbau beschleunigen und die Entwicklung der Tumorkachexie fördern.

Schlussfolgerung

Die Tumorkachexie führt zu einer fortschreitenden Einschränkung der Leistungsfähigkeit, zu Fatigue und einem Verlust der Selbstständigkeit. In der multimodalen Therapie des Kachexiesyndroms bei Krebserkrankungen stellen ernährungstherapeutische Maßnahmen einen wichtigen Baustein dar, um den klinischen Verlauf des Patienten positiv zu beeinflussen.
