Zentralvenöse Obstruktionen bei Patienten mit Dialyseshunt

Diagnostik und endovaskuläre Therapie

Zusammenfassung

Hintergrund

Wiederholte oder längerfristige zentralvenöse Katheterismen (Dialysekatheter, Schrittmacherkabel) können zu Stenosen oder Verschlüssen mediastinaler Venen führen. Die arterialisierten Flussverhältnisse im venösen Schenkel von Dialyseshunts können deren Entstehung begünstigen und führen durch ihr vermehrtes venöses Blutangebot häufig zur Demaskierung der Abflussstörung.

Ergebnisse

Bei symptomatischer zentralvenöser Obstruktion wird eine Shuntdarstellung inklusive des gesamten Abstroms mittels Angiographie empfohlen. Diese bietet die Möglichkeit zur sofortigen interventionellen Therapie. Bei geeigneter Gefäßmorphologie sollte einer perkutanen transluminalen Angioplastie wegen ihrer im Vergleich zur operativen Korrektur deutlich geringeren Invasivität der Vorzug gegeben werden. Relevante Reststenosen, periprozedurale Komplikationen und frühe oder häufige Restenosen können eine zusätzliche Stentimplantation erforderlich machen. In-Stent-Restenosen müssen häufig wiederholt Ballondilatationen zugeführt werden.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
