Qualitätsindikatoren für die Rekanalisation chronischer Okklusionen iliofemoraler Venen beim postthrombotischen Syndrom

Zusammenfassung

Hintergrund

Die Rekanalisation chronischer Okklusionen iliofemoraler Venen bei Vorliegen eines postthrombotischen Syndroms (PTS) gewinnt durch die Entwicklung neuartiger Stents zunehmend an Bedeutung. Bisher finden sich keine Empfehlungen zu Qualitätsindikatoren (QI).

Ziel der Arbeit

Ziel dieser Arbeit war es, im von der Deutschen Gesellschaft für Gefäßchirurgie und Gefäßmedizin (DGG) beauftragten Autorenkreis QI für die Rekanalisation chronischer Okklusionen iliofemoraler Venen zu entwickeln.

Material und Methoden

Nach einer bibliographischen Literaturrecherche wurden aus 15 relevanten Originalarbeiten in englischer Sprache QI definiert und diese nach einer Datenextraktionsphase auf Basis der Referenzwerte mit einem vorläufigem Zielwert versehen.

Ergebnisse

Es wurden 10 QI in Prozess- und Ergebnis-QI unterteilt. Verknüpfung der Indikationsstellung an die pathoanatomisch-morphologische Klassifikation, Durchführung einer präprozeduralen MR-Phlebographie, Verwendung venenspezifischer Stents, intraoperative Anwendung des intravaskulären Ultraschalls (IVUS), thrombotische Ereignisse innerhalb der ersten 30 Tage, technischer Erfolg, Verbesserung der Lebensqualität, Verbesserung der klinischen Scores, Offenheitsraten sowie Abheilungsraten von Ulzerationen qualifizierten sich in Anlehnung an die QUALIFY-Kriterien.

Schlussfolgerung

Die Evidenz für die Rekanalisation chronisch obstruktiver iliofemoraler Venen ist aufgrund der enormen Heterogenität der Patientenpopulationen, der ausgeprägten Variabilität der in Anwendung kommenden Stents sowie fehlender Kohortenvergleiche zwischen der konservativen und rekanalisierenden Therapie insgesamt als niedrig zu betrachten. Die aus den verfügbaren Referenzwerten generierten Zielwerte für die jeweiligen QI sind demnach Empfehlungen und bedürfen einer Validierung durch zukünftige höherwertige Evidenz.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview. CAT-Management ist ganz einfach – oder doch nicht?

Krebsassoziierte venöse Thromboembolien (CAT) haben in den vergangenen Jahren stetig zugenommen. Was hat der Anstieg mit modernen Antitumortherapien zu tun?
