Prävalenz und Schweregrad von Begleitverletzungen alkoholintoxikierter Patienten in der Notaufnahme

Zusammenfassung

Für die Risikoeinschätzung von alkoholintoxikierten Patienten hat die Erkennung von Begleitverletzungen besondere Bedeutung. In unserer Untersuchung wurden die Krankenakten von Patienten mit der primären Diagnose Alkoholintoxikation aus dem Zeitraum Januar 2003 – Dezember 2006 retrospektiv systematisch im Hinblick auf sekundär diagnostizierte Begleittraumen untersucht. Bei 80 der insgesamt 776 Patienten (10%) wurden Traumen sekundär diagnostiziert. In dieser Gruppe zeigten die Merkmale Patientenalter und Blutalkoholkonzentration signifikant höhere Werte. Die allermeisten Patienten der Traumagruppe (80%) wiesen einen niedrigen Schweregrad nach den Kriterien der „Abbreviated Injury Scale“ (AIS-Grad 1) auf, bei nur 4 Patienten wurden mäßig schwere Verletzungen (AIS-Grad 2) gefunden. Schwerwiegendere Traumen (AIS-Grade 3 und 4) hatten lediglich 2 Patienten. Diese konnten allerdings im Rahmen eines standardisierten Versorgungsschemas problemlos diagnostiziert werden. Höhere Schweregrade (AIS-Grade 5 und 6) wurden nicht gefunden, kein Patient verstarb im Rahmen der Notfalluntersuchung. Insgesamt belegen unsere Ergebnisse also ein niedriges Risiko hinsichtlich Begleitverletzungen bei primär alkoholintoxikierten Patienten.
