Neue Techniken und Entwicklungen für die Notfallversorgung von Kindern

Zusammenfassung

Verschiedene technische Neuerungen sind in den letzten Jahren für die praktische Notfallversorgung von Kindern entwickelt worden. Zugleich wurden praktische Konzepte in Tiermodellen, Simulationsstudien oder klinischen Studien bei Kindern auf ihre Wirksamkeit und Nebenwirkungen überprüft.
Die Entwicklung einer Bohrmaschine zur Anlage eines intraossären Zugangs erlaubt es, technische Schwierigkeiten und Hemmungen zu überwinden. Hiermit gelingt nach einer kurzen Schulungsphase die schnelle und sichere Anlage eines intraossären Zugangs. Der frühzeitige Einsatz der Larynxmaske als Beatmungsalternative beim schwierigen Atemweg wird in den aktuellen Leitlinien des European Resuscitation Council für Geübte empfohlen. Der Larynxtubus scheint im Kindesalter in dieser Situation schwieriger in der erfolgreichen Anwendung und kann deshalb momentan nicht empfohlen werden. Das Kompressions-Ventilations-Verhältnis wurde zugunsten der Thoraxkompressionen und einer damit verbesserten Koronarperfusion auf 15:2 verschoben. Die Verwendung von hochdosiertem Adrenalin zur Reanimation ist aufgrund erhöhter Mortalität obsolet. Eine Hypothermiebehandlung zur neuroprotektiven Therapie nach erfolgreicher Reanimation sollte erwogen werden, wobei eine individuelle Entscheidungsfindung wegen fehlender Wirksamkeitsstudien für Kinder vonnöten ist. Entscheidend für die Behandlung des septischen Schocks ist die frühzeitige und aggressive Volumengabe.
