Notfälle auf chirurgischen Stationen

Zusammenfassung

Vielen innerklinischen Todesfällen und ungeplanten Aufnahmen auf der Intensivstation geht eine langsame, aber progrediente Verschlechterung des Patienten auf der chirurgischen peripheren Station voraus, die vom medizinischen Personal nicht frühzeitig erkannt wird. Dabei sind die klinischen Zeichen einer Verschlechterung unabhängig von ihrer Ursache im Allgemeinen ähnlich und betreffen meist physiologische Basisparameter. Viele internationale Untersuchungen in chirurgischen und nichtchirurgischen Patientenkollektiven zeigen, dass mithilfe einfacher Kriterien Veränderungen dieser Basisparameter und damit eine potenzielle Gefährdung des Patienten rasch erkannt werden können. Basierend auf diesen einfachen Kriterien wurden verschiedene standardisierte Maßstäbe zur Einschätzung (Scores) entwickelt, die als praktikables Frühwarnsystem helfen können, eine Verschlechterung des Gesundheitszustandes früh zu erkennen. Nachdem ein gefährdeter Patient identifiziert wurde, sollte dieser ohne weitere zeitliche Verzögerung adäquat versorgt werden. Hierzu wurden Strukturen entwickelt, die als Frühinterventionssystem jederzeit und schnell ein kompetentes, effektives medizinisches Notfallteam („medical emergency team“, MET, „rapid response team“, RRT) am Bett des betroffenen Patienten sicherstellen. Wenn medizinische Notfallteams frühzeitig alarmiert werden, haben sie das Potenzial, katastrophale Folgen einer nicht erkannten Verschlechterung des Patienten mit konsekutivem Herz-Kreislauf-Stillstand zu verhindern. Im Gegensatz zu den in Deutschland bereits etablierten Reanimations- oder Herzalarmteams, die erst nach eingetretenem Herz-Kreislauf-Stillstand alarmiert werden, können medizinische Notfallteams bereits in der Phase der klinischen Verschlechterung aktiv werden. Mit der Etablierung standardisierter Frühwarn- und -Frühwarninterventionssysteme könnte dadurch sowohl die Patientensterblichkeit auf chirurgischen peripheren Stationen als auch die Rate an ungeplanten Aufnahmen auf die Intensivstation gesenkt werden.
