Hemorrhoids: In the past, decades spent to destroy them and, thereafter, new techniques aimed at preserving them. However, the HubBLe Trial, recently published by Brown et al. on The Lancet [
1], seems concluding that a simple, time-honored, outpatient technique aiming at destroying internal hemorrhoids is better than a new technique addressing hemorrhoid pathogenesis and symptoms while preserving the hemorrhoidal cushions. The authors have compared rubber band ligation (RBL) versus Doppler-guided hemorrhoidal dearterialization and mucopexy (using either HAL or THD device) in grades II and III hemorrhoids. This study has involved 17 UK NHS trusts, evaluating the disease recurrence rates at 1 year. …
