Therapie von Epilepsien im Kindes- und Jugendalter

Zusammenfassung

Die Behandlung der Epilepsien im Kindes- und Jugendalter hängt stark vom zugrunde liegenden elektroklinischen Syndrom ab. Bei den selbstlimitierenden Epilepsien mit fokalen Anfällen unklarer oder genetischer Ursache, wie der benignen infantilen Epilepsie, der Rolando-Epilepsie oder dem Panayiotopoulos-Syndrom besteht zunächst keine strenge Therapieindikation. Im Fall einer Therapie führen Sultiam, Levetiracetam oder Oxcarbazepin meist rasch zur Anfallskontrolle. Bei den atypischen Varianten (Pseudo-Lennox-Syndrom, „syndrome of continuous spike-waves in sleep“ (CSWS), Landau-Kleffner-Syndrom) sollten rasch Kortikosteroide eingesetzt werden. Für Epilepsien mit fokalen Anfällen aus struktureller oder unbekannter Ursache können Lamotrigin, retardiertes Oxcarbazepin oder Levetiracetam als Mittel der 1. Wahl angesehen werden. Bei schwierigem Verlauf steht eine Vielzahl an Medikamenten zur Verfügung, allerdings sollte frühzeitig die Möglichkeit eines epilepsiechirurgischen Eingriffs überprüft werden. Zur Behandlung von kindlichen Absencen gilt Ethosuximid und bei juvenilen Absencen Valproat als Mittel der Wahl. Letzteres ist auch bei der juvenilen myoklonischen Epilepsie am aussichtsreichsten. Wegen potenziell teratogener Effekte kann bei Mädchen zunächst mit Lamotrigin und ggf. Levetiracetam behandelt werden. Bei der myoklonisch-astatischen Epilepsie sollten Valproat und, wenn nötig, Lamotrigin und Ethosuximid unbedingt eingesetzt werden. Bei dieser Epilepsieform ist auch die ketogene Diät eine aussichtsreiche Behandlung. Während beim West-Syndrom in erster Linie Kortikosteroide oder Vigabatrin einzusetzen sind, besteht die Behandlung bei Lennox-Gastaut-Syndrom in der Regel in einer Polytherapie mit Valproat und verschiedenen anderen Medikamenten. Lamotrigin, Topiramat, Rufinamid, Clobazam und Felbamat sind dabei gut untersucht und werden empfohlen. Vagusnervstimulation und ketogene Diäten sind weitere Optionen. Bei Dravet-Syndrom sollte rasch nach Diagnosestellung eine Behandlung entweder mit Brom oder einer Kombination von Valproat, Clobazam und Stiripentol eingeleitet werden. Kombinationen von Bromid, Valproat und Topiramat können aussichtsreich sein. Auch bei dieser schweren Epilepsieform ist die ketogene Diät eine ernst zu nehmende Alternative. Generell erlaubt die zunehmende Zahl an zur Verfügung stehenden Antikonvulsiva in vielen Fällen eine individuell besser abgestimmte Stufentherapie, die auch Patientenfaktoren wie Geschlecht, Alter, Körpergewicht, Verhalten und Kognition berücksichtigen kann. Die Behandlung von Epilepsien im Kindesalter sollte immer die möglichst gute Entwicklung der Patienten als Ziel im Auge behalten.
