Sinnvoll bei pädiatrischen Atemwegserkrankungen?

Zusammenfassung

PCR-basierte Methoden zum Nachweis respiratorischer Erreger finden zunehmend Anwendung bei akuten Atemwegsinfektionen im Kindesalter. Vor allem sogenannte Multiplex PCRs ermöglichen mit hoher Sensitivität und Spezifität den Nachweis multipler Erreger aus einer Probe.
Die Probengewinnung ist bei Kindern im Alltag nur aus den oberen Atemwegen durchführbar. Bei viralen Infektionen decken sich Erregernachweise aus oberen und unteren Atemwegen weitgehend, so dass bei Virusnachweis z. B. eine antibiotische Therapie vermieden werden kann. Bei bakteriellen Erregern ist die PCR basierte Diagnostik nur zum Nachweis nicht kolonisierender Erreger sinnvoll, da hier eine deutlich geringere Kongruenz der Besiedlung zwischen oberen und unteren Atemwegen vorliegt.
Insgesamt ist die Wertigkeit des PCR-basierten Erregernachweises bei Atemwegsinfektionen bei Kindern noch nicht abschließend geklärt.
