Interdisziplinäre Stationen in der Viszeralmedizin – ein Erfahrungsbericht

Zusammenfassung

Traditionell erfolgte bisher die stationäre Diagnostik und Therapie von Erkrankungen des Gastrointestinaltrakts in Form einer Stafette: Diagnostik durch den Internist, Therapie durch den Chirurgen. In den letzten zwei Jahrzehnten ist die Gastroenterologie zu einem therapeutischen Fach herangereift. In einigen Bereichen hat die interventionelle Endoskopie die chirurgische Therapie abgelöst. Andererseits hat auch die Chirurgie u. a. durch die Etablierung laparoskopischer Techniken einen durchgreifenden Wandel und Fortschritt erfahren. Sowohl für die Endoskopie als auch die Chirurgie gibt es Grenzen der Methoden. Die kompetente Behandlung vieler Krankheitsbilder in der Viszeralmedizin, insbesondere von Tumorerkrankungen, erfordert inzwischen die synchrone Beteiligung mehrerer spezialisierter Disziplinen. Nur im interdisziplinären Austausch wird die für den individuellen Patienten beste Vorgehensweise möglich. Dieser Notwendigkeit trägt die Bildung interdisziplinärer Stationen Rechnung. Interdisziplinäre Stationen waren die Keimzelle enger und intensiver Zusammenarbeit von Gastroenterologen und Viszeralchirurgen, sie sind Kernbestandteile moderner Abdominalzentren.

Abstract

Traditionally the clinical treatment of gastrointestinal diseases was carried out consecutively whereby diagnostics were performed by the internal specialist and therapy by the surgeon. In last two decades gastro-enterology has matured to a therapeutic field and in some areas interventional endoscopy has replaced surgical therapy. On the other hand surgery also has changed considerably, e.g. due to the establishment of laparoscopic techniques. For endoscopic as well as surgical methods there are advantages and limits. However, competent treatment of many diseases in visceral medicine, in particular malignancies, often requires the synchronous participation of specialized internal and surgical disciplines. Only interdisciplinary cooperation allows the best approach for the individual patient. The constitution of interdisciplinary wards meets this necessity. Interdisciplinary wards are the basis of close and intensive cooperation of gastroenterologists and visceral surgeons and are essential components of modern abdominal centers.
