Subjektives und funktionelles Outcome nach akut-ligamentärer Ellenbogenluxation

Operative vs. konservative Therapie

Zusammenfassung

Hintergrund

Obgleich das konservative Vorgehen bei der akut-ligamentären Ellenbogenluxation als Standard propagiert wird, ist der Stellenwert der operativen Therapie als primäre Behandlungsoption bislang ungeklärt.

Ziel der Arbeit

Das Ziel dieser Arbeit war es, das Ergebnis nach konservativer und primär operativer Therapie akut-ligamentärer Ellenbogenluxationen mit Fokus auf die Patientenzufriedenheit und die Ellenbogengelenkstabilität zu analysieren.

Material und Methoden

In die Studie wurden Patienten nach konservativer und operativer Therapie einer akut-ligamentären Ellenbogenluxation (rein ligamentär ohne knöcherne Begleitverletzungen) eingeschlossen. Das subjektive klinische Ergebnis wurde mittels Elbow-Self-Assessment-Score (ESAS) erfasst. Als objektiver Parameter wurde eine Ultraschalluntersuchung des betroffenen sowie des kontralateralen Ellenbogens durchgeführt.

Ergebnisse

Insgesamt 20 Patienten mit einem Durchschnittsalter von 47 ± 13,1 Jahren wurden klinisch und sonographisch nachuntersucht. Der mittlere Nachuntersuchungszeitraum betrug 44 ± 18,5 Monate (Range: 15–84 Monate). Von den 20 Patienten wurden 10 Patienten konservativ und 10 primär operativ behandelt. Es zeigte sich kein signifikanter Unterschied beim Vergleich des ESAS der konservativen (91,8 ± 18,5 Punkte) mit der operativen Behandlungsgruppe (91,6 ± 15,5 Punkte; n. s.). Auch sonographisch konnte in beiden Gruppen keine signifikante Instabilität festgestellt werden.

Diskussion

Sowohl die konservative als auch die operative Therapie kann beim Vorliegen einer akut-ligamentären Ellenbogenluxation in einer sehr hohen Patientenzufriedenheit resultieren. Im Hinblick auf die Instabilität korreliert die subjektive Wahrnehmung nicht zwangsläufig mit dem sonographisch erhobenen Befund.
