Zusammenfassung
Das 12-Kanal-Oberflächen-EKG ist von großer Bedeutung für die Diagnostik von Rhythmusstörungen und für das akute Koronarsyndrom. Supraventrikuläre Tachykardien (SVT) wie Sinustachykardien, atriale Tachykardien, AV-Knoten-Reentry-Tachykardien und Tachykardien, die durch akzessorische Leitungsbahnen bedingt sind, führen in der Regel zu Tachykardien (Kammerfrequenz >100 min) mit schmalen QRS-Komplexen (QRS-Breite <0,12 s). Besondere Bedeutung kommt der Relation von P-Welle und folgendem QRS-Komplex zu sowie einem elektrischen Alternans als Hinweis für eine akzessorische Leitungsbahn. Tachykardien mit breitem QRS-Komplex (QRS-Breite ≥0,12 s) können bei SVT mit aberrierender Leitung, bei SVT mit Schenkelblock oder Kammertachykardien (KT) auftreten. Bei Tachykardien mit breitem QRS-Komplex sprechen AV-Dissoziation, negative oder positive Konkordanz in V1–V6, eine „Kerbe“ in V1 und ein qR-Komplex in V6 bei Linksschenkelblockmorphologie für eine KT, eine R/S-Relation <1 in V6 bei Rechtsschenkelblockbild ebenfalls für die Diagnose einer KT. Das Oberflächen-EKG hat bei richtiger und systematischer Analyse eine Sensitivität und Spezifität für die korrekte Diagnose einer SVT oder KT von über 95%. Das 12-Kanal-Oberflächen-EKG erlaubt die Lokalisation proximaler Koronararterienverschlüsse in 97% durch Bestimmung des Vektors der ST-Strecke im 12-Kanal-Oberflächen-EKG.
Abstract
The surface electrocardiogram (ECG) is an important diagnostic tool for the diagnosis of arrhythmias and acute coronary syndrome. Supraventricular tachycardias (SVT) are paroxysmal tachycardias as are sinus tachycardia, atrial tachycardia, AV nodal reentry tachycardia, and tachycardia due to accessory pathways. All SVT are characterized by a ventricular heart rate >100/min and small QRS complexes (QRS width <0.12 s) during tachycardia. It is important to analyze the relation between P wave and QRS complex to look for an electrical alternans as a leading finding for an accessory pathway. Wide QRS complex tachycardias (QRS width ≥ 0.12 s) occur in SVT with aberrant conduction and SVT with bundle branch block or ventricular tachycardia (VT). In broad complex tachycardias, AV dissociation, negative or positive concordant pattern in V1–V6, a notch in V1 and QR complexes in V6 in tachycardias with left bundle branch block morphologies are findings indicating VT. In addition, an R/S relation <1 in V6 favors VT when right bundle branch block tachycardia morphologies are present. By analyzing the surface ECG in the right way with a systematic approach, the specificity and sensitivity of correctly identifying a SVT or VT can be raised by >95%. The 12-lead surface ECG allows the coronary culprit lesion to be located in 97% due to determination of the 12-lead ST segment deviation score.
