Résumé
Un second épisode d’hémolyse intravasculaire après prise de l’association artéméther-luméfantrine est décrit chez un enfant congolais de huit ans. Le patient a eu des antécédents d’hémoglobinurie après un traitement à la quinine. Il a reçu un médicament combinant l’artémisine à la luméfantrine pour traiter un paludisme. Nous n’avions pas noté de déficience en glucose-6-phosphate-déshydrogénase (G6DP), ni d’anomalies de l’hémoglobine. Le sepsis a été exclu. Une anémie de type hémolytique a été observée avec un taux d’hémoglobine à 5,6 g/dl, un test direct à l’antiglobuline négatif et un taux de LDH à 893 UI/l. La parasitémie était faible. L’évolution a été favorable après hyperhydratation. Des épisodes de fièvre bilieuse hémoglobinurique ou des réactions immunoallergiques ont été décrits chez des patients ayant pris de la quinine, de la méfloquine et de l’halofantrine. Quelques rares cas l’ont été après prise de luméfantrine. Cette observation a une implication thérapeutique majeure: les aminoalcools sont une contre-indication absolue chez les patients ayant eu des antécédents d’hémolyse à la suite de la prise d’un aminoalcool.
Abstract
A second haemolytic crisis of blackwater fever (BWF) following a combination of artemether-lumefantrine intake, in an 8-year-old Congolese boy is reported. The patient had a history of BWF after quinine intake. He was given artemether-lumefantrine treatment for malaria. He was free from G6PD deficiency and abnormal haemoglobin. Sepsis was eliminated. Haemolysis was noted with 5.6 g/dl of haemoglobin, negative direct antiglobulin test, and LDH at 893 IU/l. Low-level Plasmodium falciparum was found. The outcome was favourable with rehydration. BWF has been described with quinine, mefloquine and halofantrine. Several case reports have been published of haemolysis after lumefantrine, but it is quite rare. This case has a major therapeutic implication: aryl-amino-alcohol should be strictly contraindicated in patients with history of BWF with aryl-amino-alcohols intake.
