Women and the elderly are referred less frequently to cardiac rehabilitation programs, even though they derive benefit from them (45,813–816). Health care systems should consider instituting processes that encourage referral of appropriate patients to cardiac rehabilitation/secondary prevention programs (for example, the use of standardized order sets that facilitate this, such as the AHA “Get with the Guidelines” tools).
