Serum gonadotropin and steroid patterns during the normal menstrual cycle

Abstract

Follicle-stimulating hormone (FSH), luteinizing hormone (LH), progesterone, and estradiol were measured in aliquots of the same serum samples obtained daily from a group of 10 women throughout a normal cycle. Measurement of each of these hormones was performed by sensitive and specific competitive binding techniques. The patterns and mean levels of each of these hormones in this group of unselected ovulatory cycles is in general agreement with the results obtained by combining the findings of other investigators. In the early follicular phase, there are higher levels of FSH, followed by rising levels of estradiol which reach a peak at mid-cycle. A peak in LH occurs on the same or subsequent day, and it in turn is followed by a rise in progesterone and a secondary rise in progesterone and in estradiol. Both these steroids then fall a few days prior to menstruation.

    Although the underlying mechanisms regulating the changes in FSH isoforms during puberty remain unclear, changes in GnRH secretory pattern, circulating concentrations of gonadal steroids, and endocrine or paracrine effects of protein regulators are all suspects (Ulloa-Aguirre et al., 1995). The reduction in circulating levels of estradiol, progesteroneand inhibin A in the beginning of the menstrual cycle due to the demise of the corpus luteum reduces the inhibitory effects on the pituitary and allows FSH to rise in the early follicular phase and stimulate follicular growth and selection (Lee et al., 1988; Mishell et al., 1971; Yding Andersen, 2017). At approximately Day 7 of the menstrual cycle, when selection of the dominant follicle occurs, FSH concentrations have already peaked and started to decline, with a continued slow decline until ovulation occurs (Lee et al., 1988; Yding Andersen, 2017).
