2009, Journal of Renal NutritionCitation Excerpt :In the present investigation, we failed to show a significant effect of 6 months of OTC fish-oil supplementation on Lp(a) status in an ESRD population, although results from this study support findings from other studies suggesting that African Americans have higher Lp(a) concentrations than persons of Caucasian decent.30 Although some studies suggest that fish-oil supplementation can mediate Lp(a) expression,18,19 our findings concur with a study of a similar design by Svensson et al.21 In that double-blind controlled trial, 206 HD patients were randomized to consume either 1.7 g/day of n-3 fatty acid (EPA, 45%; DHA, 37.5%) or olive-oil capsules for 3 months. Participants were evaluated at a clinical examination and via blood sampling at baseline and after 3 months of treatment.

Therefore, the serum Lp(a) reduction observed here may have resulted from lowering the hepatic synthesis of Lp(a) by 1,3-DAG, similar to antihypertriglyceridemic agents such as nicotinic acid [46] and fish oil [47], and promoting the catabolism of Lp(a) by PS in a synergistic manner. However, additional studies will be required to address this issue.

Recently, some intervention studies based on the effect of fish oil rich in n-3 polyunsaturated fatty acids (PUFA) on Lp(a) concentrations have been conducted, but no clear indications have been obtained [12–15]. Lp(a) concentrations are thought to be strongly under genetic control at the concentration of biosynthesis of the apo(a) protein, which is encoded by the LPA locus, so allelic differences at LPA are responsible for the variations in Lp(a) phenotype [16].
