B12 associated psychosis could manifest as suspiciousness, persecutory or religious delusions, auditory and visual hallucinations, tangential or incoherent speech, and disorganization in the thought process (Hutto, 1997). B12 deficiency can sometimes present with a fluctuating clinical picture characterized by affective changes, anxiety and restlessness, alteration in sensorium which could be collectively termed as atypical psychosis (Phillips & Kahaner, 1988).
