The neonatal immune system matures rapidly after birth to acquire an ability to recognize and defend against potentially harmful pathogens, to tolerate and even promote commensal microbes, and to accept relatively harmless environmental antigens and dietary macromolecules. Although many components of the immune system perform nearly at par with adults, other branches are still maturing. The teleological significance of some of these alterations is still being elucidated. In this chapter, we describe the various arms of the neonatal immune system and compare these with their counterparts in adults.

The developing neonate is exposed to a plethora of antigens and must evolve innate and adaptive immune responses to combat pathogenic microorganisms and, at the same time, develop tolerance to self-antigens and commensal microbes. While some components of the immune system are functionally at par with adults, immaturity in others may result in a state of immunodeficiency in the neonate. This chapter highlights the qualitative and quantitative differences in the distribution and function of leukocyte populations that together shape the immune responses in the fetus, neonate, and adult. Knowledge of these differences may enable a better understanding of the inherent susceptibility of the neonate to various infections.
