2023, Surgical Pathology ClinicsCitation Excerpt :Atypical parathyroid adenomas were usually less pronounced in size but could be adherent to surrounding tissues (without overt signs of invasion).31 Microscopically, a diagnosis of atypical parathyroid adenoma was rendered with the observance of worrisome features including mitoses, desmoplastic reaction, nuclear atypia, necrosis, monotonous sheet-like or trabecular growth, fibrosis, and tumor cells in the capsule.4,30–35 Histologic signs of invasion (perineural, intravascular, lymphatic, or pericapsular) or metastasis upgraded the diagnosis to a parathyroid carcinoma.30

2011, Radiologic Clinics of North AmericaCitation Excerpt :Adenomatous and hyperplastic parathyroid tissue show more avid uptake of 99mTc-sestamibi although initially this activity can be obscured by uptake from the adjacent thyroid gland. Subtraction techniques that use a thyroid agent such as 99mTc-pertechnitate or 123I coregistered with a 99mTc-sestamibi image attempt to improve conspicuity of parathyroid tissue by subtracting out the adjacent thyroid uptake.52,53 The observation that abnormal parathyroid tissue commonly retains 99mTc-sestamibi longer than thyroid tissue has led to the widespread use of delayed imaging to improve upon the conspicuity of hyperfunctioning parathyroid tissue.
