Surgical management of arterial lesions of the thoracoabdominal aorta

Four patients with symptomatic lesions of the thoracoabdominal aorta are described. Exposure of the lower half of the thoracic aorta and the entire abdominal aorta was obtained using a modified left thoracoabdominal (retroperitoneal) approach. Three aneurysms were excluded and composite graft reconstruction was performed. One occlusive lesion was removed by endarterectomy. All patients were relieved of symptoms and three are long-term survivors.
