IN-VITRO SYNTHESIS OF ANTI-ACETYLCHOLINE-RECEPTOR ANTIBODY BY THYMIC LYMPHOCYTES IN MYASTHENIA GRAVIS

Abstract

Cultures of thymic tissue or thymic lymphocytes from nine patients undergoing thymectomy for myasthenia gravis were examined for evidence of anti-acetylcholine-receptor-antibody synthesis. One of four thymic tissues and three of five thymic lymphocyte preparations synthesised this antibody.

    V−30 was also strongly correlated with the histologic volume, which supports the reliability of V−30 as a reflection of the actual thymus volume. The ability to produce AChRAb in the thymus is the basis for choice of extended thymectomy for MG, although it is appropriate to regard MG as a systemic disease because other sites may also produce AChRAb.23,24 Establishment of the volume of the lesion associated with MG or the thymic contribution to MG may help to determine treatment strategy for MG and to elucidate the etiology of MG.

    This heterogeneity implies a polyclonal response of thymic B cells to various antigens. Such specificities have previously been documented among thymic MG B cells for tetanus toxoid [19], influenza virus [17,56] and striational muscle antigens upon stimulation with pokeweed mitogen (PWM) [39]. In contrast, AChR antibody production is rarely increased by PWM stimulation [29].
