THE COMMON MIGRAINE ATTACK MAY NOT BE INITIATED BY CEREBRAL ISCHAEMIA

Abstract

Regional cerebral blood-flow was measured in the resting state and during the development of common migraine attacks induced by red wine. The tomographic xenon-133 method was used in four studies, the intra-arterial xenon-133 method in one study, and the xenon-133 inhalation method in the remaining three studies. No significant focal or global reduction of cerebral blood-flow was found at any time. This contrasts with previous findings of reduced cerebral blood-flow in classical migraine. It is suggested that these two forms of migraine have a different pathophysiology.

    In 1944, Aristides Leão described cortical spreading depression (Leão, 1944). Olesen et al. found that regional cerebral blood flow decreased during migraine aura, followed by hyperperfusion during headache, and finally, a return to normal blood flow after the headache subsides (Olesen et al., 1981). In the 1980s, the neurogenic inflammation theory was proposed based on the observation that proinflammatory mediators are released, along with vasodilation during migraine attacks (McCulloch et al., 1986; Moskowitz and Cutrer, 1993).

    This might be another reason for the diminished, non-significant association between air pollution and migraine on high-temperature days when we defined high-temperature days using a 99th percentile temperature (28.7 °C) (Fig. 4). Previous studies have found that reduced regional cerebral blood flow is prominent in patients with aura, but not in patients without aura (Olesen et al., 1981a; Olesen et al., 1981b). A clinical study has found different effectiveness of pharmacotherapy according to subtype of migraine (with and without aura) (Hansen et al., 2015).
