Patientes
Trente-cinq patientes âgées de 22 à 46 ans (moyenne d’âge de 31 ans), suspectes d’endométriose (n = 13) ou ayant un diagnostic connu d’endométriose (n = 22), ont bénéficié d’une exploration pelvienne par IRM entre septembre 2004 et juillet 2007.
Les principaux symptômes présentés étaient : douleurs pelviennes chroniques (46 %), infertilité primaire (46 %), dyspareunie profonde (37 %), dysménorrhées (26 %), signes digestifs (ténesme, douleur à la défécation) (14 %) et signes urinaires à type de
Résultats chirurgicaux
La cœlioscopie confirmait l’existence de lésions évocatrices d’endométriose chez les 35 patientes. Elle diagnostiquait des lésions d’endométriose profonde des ligaments utérosacrés (n = 10), du torus uterinum (n = 9), de la paroi du rectum ou du colon sigmoïde (n = 11), du cul-de-sac recto-vaginal (n = 9), du septum recto-vaginal (n = 6), de la paroi vésicale (n = 4) ou du cul-de-sac vaginal postérieur (n = 2) chez 25 patientes. Dix patientes ne présentaient pas de lésions d’endométriose
Discussion
L’endométriose est une pathologie dont le diagnostic clinique est souvent difficile et retardé compte tenu de la non-spécificité des symptômes. Le diagnostic de certitude repose sur l’histologie des prélèvements, mettant en évidence des glandes endométriales en position ectopique. L’examen « gold standard » pour le diagnostic et la classification de l’endométriose est la cœlioscopie, permettant d’établir la sévérité de celle-ci selon le score de l’AFS r. Conclusion
Selon les données de la littérature, l’IRM est le meilleur examen d’imagerie pour le bilan des lésions d’endométriose profonde. Notre étude a montré de bonnes sensibilités dans la détection des localisations vésicales, digestives et du cul-de-sac de Douglas ainsi que de bonnes spécificités dans l’ensemble des localisations. Le manque de sensibilité pour la détection des atteintes des ligaments utérosacrés, du cul-de-sac vaginal postérieur et de la cloison recto-vaginale montre la difficulté de
