Introduction
La maladie cœliaque est une pathologie qui s’associe rarement à des complications thrombo-emboliques. La thrombose veineuse cérébrale n’a jamais été décrite chez des patients ayant une maladie cœliaque.
Observation
Nous rapportons une observation d’une fille âgée de 21 ans ayant des antécédents de maladie cœliaque sous régime sans gluten, hospitalisée en réanimation pour état de mal convulsif en rapport avec un ramollissement cérébral secondaire à une thrombose veineuse cérébrale. L’enquête étiologique de cette complication thrombo-embolique révèle un déficit en protéine S.

Abstract
Introduction
Celiac disease is a pathology which is rarely associated with thrombosis complications. Cerebral vascular thrombosis has never been described in patients with a celiac disease.
Observation
We report an observation of a 21-year-old girl with a history of celiac disease who was hospitalized in the intensive care unit for convulsive status epilepticus secondary to a cerebral venous thrombosis. The etiologic investigation of this thrombo-embolic complication revealed protein S deficit. Our patient improved under symptomatic treatment.
Comment
This original observation confirms that celiac disease can be associated with cerebral venous thrombosis. (J Mal Vasc 2005; 30: 228-230)

Intracardiac thrombosis during celiac disease
2014, Journal des Maladies VasculairesDes événements thrombotiques sont fréquemment rapportés au cours de la maladie cœliaque. Ces thromboses sont souvent de siège insolite et touchent essentiellement les veines hépatiques. La survenue de thrombose intracardiaque au cours de la maladie cœliaque n’a jamais été décrite. Nous rapportons l’observation d’un patient âgé de 32 ans, suivi pour maladie cœliaque avec mauvaise observance du régime sans gluten, et qui a présenté un accident vasculaire cérébral ischémique révélant un thrombus intracardiaque simulant à l’imagerie un myxome multiple. L’examen anatomopathologique de la pièce de résection a permis de rétablir le diagnostic. Le bilan de thrombophilie a révélé un déficit profond en protéines C et S. L’évolution était favorable sous traitement anticoagulant et régime sans gluten.
Thrombotic events occurring in the course of celiac disease are frequently reported in the literature. The localization is often unusual, mainly affecting the hepatic veins. To our knowledge, this is the first report of intracardiac thrombosis occurring in a patient with celiac disease. A 32-year-old patient with celiac disease adhered poorly to his gluten-free diet. He suffered an ischemic stroke revealing an intracardiac thrombus, which, on radiological imaging, simulated a multiple myxoma. Histological examination of the resected tumor enabled the correct diagnosis. Biological findings revealed severe protein C and S deficiency. The patient improved with anticoagulant therapy and gluten-free diet.
Association of celiac disease and cerebral venous thrombosis: Report of two cases
2013, Journal des Maladies VasculairesLes thromboses veineuses au cours de la maladie cœliaque sont signalées dans la littérature, surtout chez l’adulte. Des facteurs de risque thrombophiliques ont été identifiés. Nous rapportons deux observations.
Premier cas : patiente âgée de 63 ans hospitalisée pour thrombose veineuse cérébrale sur maladie cœliaque. Le bilan a révélé une hyperhomocystéinémie et des antiphospholipides de type β2GPI. Second cas : patiente de 19 ans admise pour troubles de la conscience en rapport avec une thrombose du sinus longitudinal. Le bilan retrouve un déficit en protéine S ; l’examen anatomopathologique des biopsies jéjunales conclut à une atrophie villositaire ; les anticorps antiendomysium et antigliadine sont positifs. Sous anticoagulants, l’évolution est favorable dans les deux cas.
Si les thromboses veineuses associées à la maladie cœliaque sont de siège insolite, la localisation cérébrale reste exceptionnelle. La recherche de facteurs de risque est indispensable chez ces patients. Le régime sans gluten reste le traitement essentiel de la maladie cœliaque.

To report two cases of cerebral venous thrombosis in celiac disease patients and to determine associated thrombophilic factors. Case 1: a 63-year-old woman was admitted for cerebral venous thrombosis. Her past history included celiac disease which was untreated for the last 10 years. Biological data showed moderate hyperhomocysteinemia and positive reaction for antiphospholipid antibodies. Case 2: a 19-year-old girl was hospitalized for seizures and impaired consciousness. MRA demonstrated longitudinal sinus thrombosis with parietal infarction. Biological findings showed: protein S deficiency and positive reaction for antiendomysium and antigliadin antibodies. Gastroesophageal endoscopy with biopsy showed villous atrophy. Both patients improved under anticoagulant treatment.
The sites of associated venous thrombosis are generally unusual in celiac disease, yet the cerebral localization is only exceptionally reported. It is essential to search for risk factors. Gluten-free diet remains the mainstay treatment for patients with celiac disease.
Thrombosis of the superior vena cava revealing a deficiency of the protein S. About one case

Elle se manifeste de manière progressive par un syndrome cave supérieur (SCS). Nous rapportons l’observation d’une patiente âgée de 22 ans, sans habitudes toxiques ou d’aphtose bipolaire, qui présentait depuis trois mois un SCS. Les bilans radiologiques et biologiques confirmaient la thrombose de la veine cave supérieure et un déficit isolé de la protéine S pour lequel la malade fut mise sous traitement anticoagulant avec une bonne amélioration clinique et radiologique. À travers cette observation, les auteurs rapportent une étiologie rare de la thrombose de la veine cave supérieure qui est le déficit isolé en protéine S et proposent une revue de la littérature.
Thrombosis of the superior vena cava is attached to several causes including deficiency of the protein S. It occurs gradually by a superior vena cava syndrome. We report a patient aged 22 years, no toxic habits or bipolar aphthosis, which presented for 3 months a superior vena cava syndrome. The radiological and biological assessments confirmed thrombosis of the superior vena cava and a deficit of the protein S for which the patient was put under anticoagulant therapy with good clinical and radiological improvement. Through this observation, the authors report a rare cause of thrombosis of the superior vena cava which is the isolated protein S deficiency and provide a review of the literature.
Adult celiac disease with thrombosis: A case series of seven patients. Role of thrombophilic factors
2011, Revue de Medecine InternePlusieurs cas de thrombose ont été décrits dans la maladie cœliaque. Dans ce travail, nous avons analysé les caractéristiques cliniques et les facteurs de thrombophilie de sept patients adultes ayant présenté une thrombose associée à une maladie cœliaque.
Il s’agit d’une série rétrospective de sept observations de thrombose parmi une cohorte de 87 patients ayant une maladie cœliaque révélée à l’âge adulte. Les facteurs de thrombophilie ont été répertoriés et une recherche d’anticorps antiphospholipides a été faite de manière rétrospective sur le sérum des sept patients.
La prévalence globale des thromboses était de 8 %, celle des thromboses spontanées de 5,7 %, avec des thromboses veineuses (n = 5) ou artérielles (n = 1) ou l’association des deux (n = 2). Il s’agissait de six femmes et d’un homme d’âge moyen 44,8 ans au moment de la thrombose. Les manifestations thrombotiques précédaient le diagnostic de maladie cœliaque dans quatre cas. Dans trois cas, la thrombose était de siège inhabituel : veine porte (n = 2), veine splénique (n = 1). Des facteurs de risque de thrombose étaient identifiés dans six cas et pouvaient tous être reliés à la maladie cœliaque : hyperhomocystéinémie (n = 1), déficit en protéine C et S (n = 3) liés à une carence en vitamine K et anticorps antiphospholipides (n = 2).
Des facteurs de risque de thrombose peuvent être acquis au cours de la maladie cœliaque par carence vitaminique ou anticorps antiphospholipides. Il faut rechercher ces facteurs, les corriger, voire mettre en place une prophylaxie thromboembolique.
Thrombotic events may occur in celiac disease. In this study, we analyzed clinical features and risk factors for thrombosis in seven patients who had celiac disease and thrombosis.
We retrospectively studied 87 patients with adult celiac disease and identified seven cases of thrombosis. We searched if risk factors for thrombosis were identified and tested retrospectively antiphospholipid antibodies on the serum.
In our study, the global prevalence of thrombosis was 8 %, and 5.7 % for spontaneous thrombosis, with venous thrombosis (n = 5) or arterial thrombosis (n = 1) or both (n = 2). The seven patients consisted in six women and one man with a mean age of 44.8 years at time of thrombosis. Thrombotic events occurred before the diagnosis of celiac disease in four cases. In three cases, venous thrombosis was in unusual sites: portal (n = 2), splenic vein thrombosis (n = 1). In six cases, we identified risk factors for thrombosis, which could be linked to celiac disease: hyperhomocysteinemia (n = 1), protein C and S deficiency due to vitamin K deficiency (n = 3) and antiphospholipid antibodies (n = 2).
Such risk factors for thrombosis should be identified in patients in adult celiac disease in order to correct them and add a thromboembolic prophylaxis.
