Summary

Objectives

The purpose of this work was to evaluate the feasibility and outcome of elective laparoscopic cholecystectomy as a day-case procedure in a French university hospital.

Methods

Since the creation of a surgical day-care centre in 1999, patients without severe chronic disease and anticoagulant therapy were selected for elective laparoscopic cholecystectomy. They were admitted and operated on in the morning hours and discharged after a double check by the surgeon and an anaesthetist 4 to 6 hours later. They were contacted by telephone the day subsequent to surgery and were seen in the outpatient unit 8 to 10 days after.

Results

Two hundred eleven laparoscopic cholecystectomies were performed in day-care surgery from January 1999 to December 2005. The proportion of day-case management increased during the six-year period from 32% to 53%. Eighteen percent of patients had an overnight admission. The overall complication rate was 1.8%. None of the patients had an emergency readmission. Incapacity duration went from 1 to 15 days.

Conclusion

These results suggest that laparoscopic cholecystectomy can be routinely performed as a day-case procedure.

Objectifs

L’objectif de cette étude était d’évaluer la faisabilité et les résultats de la cholécystectomie par laparoscopie en chirurgie ambulatoire dans un centre hospitalo-universitaire français.

Méthodes

Depuis l’ouverture d’un centre de chirurgie ambulatoire en 1999, des malades sans maladie chronique sévère ni traitement anticoagulant ont été sélectionnés pour la réalisation d’une cholécystectomie laparoscopique élective. Ils étaient admis et opérés le matin, et leur sortie était doublement validée par le chirurgien et un anesthésiste 4 à 6 heures plus tard. Ils étaient contactés par téléphone le lendemain de l’intervention et revus en consultation 8 à 10 jours après l’intervention.

Résultats

Deux cent onze cholécystectomies par laparoscopie ont été réalisées en chirurgie ambulatoire de janvier 1999 à décembre 2005. La proportion de malade pris en charge en chirurgie ambulatoire par rapport au nombre total de cholécystectomies électives a augmenté de 32 % à 53 % durant cette période de six ans. Dix-huit pour cent des malades ont dû être hospitalisés pour la nuit. Le taux global de complications était de 1,8 %. Aucun des malades n’a eu de réadmission en urgence. La durée d’arrêt du travail allait de 1 à 15 jours.

Conclusion

Ces résultats suggèrent que la cholécystectomie par laparoscopie peut être réalisée en ambulatoire en pratique courante.
