Abstract

The magnetic resonance imaging findings of histologically-proven renal and pancreatic granulocytic sarcomas are demonstrated in two patients. Granulocytic sarcomas were moderately hypointense on T₁-weighted images, mildly hyperintense on T₂-weighted images, and enhanced in a diminished fashion on early and late postgadolinium T₁-weighted images.

Rare isolated pancreatic myeloid sarcoma in a pediatric patient – A case study with literature review

Myeloid sarcoma (MS) is an extramedullary hematopoietic tumor of immature myeloid cells. It is more common in males, and may present across a wide range of age groups. It rarely occurs in isolation and is chiefly associated with AML, of which it may be the first presentation. Most patients with MS will progress to AML within one year of diagnosis, but this can be delayed with early detection and treatment. Isolated MS is very rare, but can occur at any site in the body. MS in the pancreas is uncommon and can simulate the symptoms of pancreatitis or adenocarcinoma, making this a challenging diagnosis. Here, we present a case of MS diagnosed in a 12-year-old boy with no known history of AML. To our knowledge, this is the first reported case of isolated pancreatic myeloid sarcoma in Saudi Arabia.
FLT3-internal tandem duplication in a pediatric patient with t(8;21) acute myeloid leukemia

Patients diagnosed with t(8;21)-acute myeloid leukemia (AML) are currently considered to have good prognoses, but about half of these patients relapse. FLT3-internal tandem duplication (ITD) is generally thought to be strongly associated with poor prognosis in AML, but is rarely reported in patients with t(8;21)-AML. Expression of the neural cell-adhesion molecule (CD56) is also associated with a significantly shorter complete remission duration and survival in patients with t(8;21)-AML. Patients with t(8;21)-AML expressing CD56 have been reported to exhibit a higher incidence of granulocytic sarcoma (GS), and t(8;21)-AML with GS results in a less favorable prognosis than AML with this translocation alone. Here, we report on a 15-year-old girl with t(8;21)-AML having both CD56 expression and FLT3-ITD. This patient underwent unrelated donor bone marrow transplantation and achieved complete remission, but thereafter presented with obstructive jaundice caused by GS compression of the common bile duct without bone marrow invasion at relapse. Autopsy revealed multiple nodules of the stomach membrane and invasion into the head of the pancreas. For earlier detection of relapse, we suggest that it would be useful to examine existence of GS in CD56-positive t(8;21)-AML patients at diagnosis and hematologic remission. Even though t(8;21)-AML is less likely to co-occur with FLT3-ITD in pediatric patients, this report suggests that prognostic factors, including FLT3 and KIT genes and the surface marker CD56, should be analyzed in these patients.
Chloroma of the adrenal gland and the peritoneum: CT diagnosis of an unusual presentation of acute myeloid leukemia

Chloroma (syn: granulocytic sarcoma) is an unusual solid extramedullary manifestation of acute myeloid leukemia (AML) consisting of deposits of myeloid precursor cells. Most chloromas develop during the course of leukemia or following. In rare cases, they are the first and sole manifestation of AML. In this specific condition, the diagnosis of chloroma can, therefore, be difficult to differentiate from lymphoma or other malignancies using clinical, radiological and even histopathological methods. Chloromas can involve any part of the body, however, bone, periosteum, soft tissue, lymph nodes and skin are the most common sites. Imaging appearance of chloromas is not specific but the diagnosis should be strongly considered in patients with a history of leukaemia who develop a solid tumor.
This report describes the CT appearance of an unusual case of chloroma in a 70-year-old man presenting with a suprarenal mass and multiple peritoneal involvements, but no haematological abnormalities.
