Currently, the literature demonstrates equivocal evidence [11,18], and most studies have reported on retrospective case series with insufficient patient numbers [9,10,14–16], or biomechanical studies using synthetic or cadaveric bones [20,24–28]. To our knowledge, there is no prospective randomized controlled trial that evaluates the optimal technique for the fixation of Vancouver B1 periprosthetic femoral fractures.

Plate fixation [5,14–19], cerclages [6,11,13,19–24], a combination of a non-locking plate with an allograft strut [12,20,25–26] and even external fixation have been described [27]. Moreover, the technique for fracture fixation depends on whether the periprosthetic fracture occurred around a cemented or an uncemented stem.
