Abstract

The incidence of cancer during pregnancy is a rare phenomenon and is estimated to occur in 1:1000 pregnancies. This co-existence is likely to rise since the delay of childbearing to the later reproductive age is nowadays more common in families. The most frequent malignancies diagnosed during pregnancy are breast cancer, cervical cancer, haematological malignancies (lymphomas and acute leukaemias) and melanoma. Less common tumours are gastrointestinal, urological and lung cancers [1].

Table 3 summarizes the reported 193 patients diagnosed with NEOC, and treated with chemotherapy during pregnancy. Among 145 documented cases of germ cell tumours, histopathology was compatible with EST in 52 patients [1,4,40,49,76,77,79,81,87,88], dysgerminoma in 45 [4,8,40,75,80,87], immature teratoma in 24 [4,40,76,82,87,89], whereas mixed elements were revealed in 13 patients [1,40,78,87], respectively. Platinum-bleomycin based chemotherapy was administered in 68 patients.
The management of gestational ovarian cancer can be challenging because of the risk of fetal wastage, and the possibility of treatment-related complications to the fetus; it is based on insufficient data from retrospective studies and case series. Here, a literature review of the diagnostic and surgical approaches to the gestational ovarian cancer has been performed; moreover, data on safety of chemotherapeutic treatments in pregnancy, including both oncologic and fetal outcomes, have also been reviewed. Up to now, 193 cases of ovarian cancers during pregnancy have been reported in the English literature. Treatment of ovarian malignancies during pregnancy depends on histology, stage, and gestational weeks. When possible, surgical excision is indicated, and fertility-sparing surgery can be offered to stage I epithelial ovarian tumours (EOC), germ cell ovarian, or sex-cord stromal ovarian tumours. Neoadjuvant and/or adjuvant chemotherapy for advanced ovarian tumours is indicated as in non-pregnant women. Administration of chemotherapy after the first trimester, can cause fetal growth restriction, while being seemingly safe. The therapeutic approach of ovarian cancer in pregnancy should be individualized and intended in specialized centers.

L’incidence du cancer bronchopulmonaire (CBP) pendant la grossesse est faible mais tend à augmenter dans les pays industrialisés du fait de l’augmentation du tabagisme chez les jeunes femmes et de l’âge tardif de la première grossesse. Dans la majorité des cas, il s’agit d’une maladie d’emblée métastatique au pronostic très sombre. La prise en charge diagnostique et thérapeutique est délicate, nécessitant de prendre en compte l’âge gestationnel du fœtus, la gravité du pronostic maternel, la toxicité fœtale des traitements, mais également l’aggravation du pronostic maternel et le risque de transmission de cellules cancéreuses au fœtus en l’absence de traitement. Les conséquences psychologiques et les implications éthiques d’une telle situation sont maximales pour les patientes et l’équipe soignante. Nous proposons une revue de la littérature sur le sujet, abordant l’épidémiologie, les caractéristiques cliniques, les possibilités de traitement, reposant en partie sur l’analyse des autres localisations de cancers, et le pronostic de cette affection aussi bien au plan maternel que fœtal. Enfin, il faut rappeler la nécessité d’une contraception efficace chez les patientes en âge de procréer atteintes d’un CBP.
The incidence of lung cancer during pregnancy is very low, but it is becoming more frequent in industrialized countries both because of the increase in smoking in young women and because women are becoming pregnant later in life. Usually, the cancer has a poor prognosis due to the presence of metastatic disease at the time of diagnosis. Diagnosis and management are delicate, and should deal with the gestational age, the maternal prognosis, the fetal toxicity of treatments, but also with the worsening of maternal prognosis and the risk of neoplastic cells being transmitted to the fetus in case of delayed treatment. Psychological and ethical considerations complicate the decision process. We present a review of the epidemiology, clinical characteristics, management, and prognosis concerning lung cancer during pregnancy. Finally, it is important to remember that young women with lung cancer should be advised to use a reliable form of contraception.
