La abundante bibliografía nos demuestra que la familia tiene un papel fundamental en la recuperación de los enfermos hospitalizados, y los pacientes de las Unidades de Cuidados Intensivos (UCI) no son una excepción. Varios autores han demostrado que un ambiente desangustiado y tranquilo ayuda en el proceso de cuidado. Por esto, es necesario plantearse las necesidades de las familias para mejorar su bienestar, de esta forma su actitud ayudará en la recuperación del enfermo. El personal asistencial tiene que conocer dichas necesidades y suplir el déficit en la medida posible. El primer estudio que valora esta cuestión utilizó el cuestionario CCFNI (Critical Care Family Needs Inventory), que mide el grado de importancia que los familiares otorgan a las necesidades de seguridad, apoyo, información, comodidad y proximidad. El objetivo de este estudio fue detectar el grado de importancia que dan los familiares de los pacientes ingresados en la UCI del Hospital Dr. Josep Trueta de Girona, empleando el cuestionario CCFNI. Se entrevistó a todos los familiares de los pacientes ingresados en la UCI del hospital durante el otoño del año 2003 y se compararon las necesidades consideradas más importantes con las variables sociodemográficas de los familiares.
La muestra total fue 132 personas entrevistadas. La necesidad considerada más importante fue la seguridad con un 50,3%, seguida de la necesidad de apoyo con un 47,2%. En los resultados se observaron algunas diferencias dependiendo de la edad y estudios del encuestado y según el diagnóstico y procedencia del ingreso. Los datos obtenidos no discrepaban con la bibliografía revisada. Todo el estudio fue muy valioso para los profesionales asistenciales de la unidad y se tuvieron en cuenta para mejorar las deficiencias de la UCI del hospital de Girona.
The abundant bibliography shows us that the family plays a fundamental paper in the recovery of the hospitalized patients, and the patients in the intensive care units (ICU) are no exception. Several authors have demonstrated that a stress-free and calm setting helps in the care process. Thus, the family needs must be considered to improve their well-being. In this way, their attitude will help in the patient's recovery. Health care personnel must be aware of these needs and make up for their deficit as far as possible. The first study that evaluated this question used the CCFNI questionnaire (Critical Care Family Needs Inventory) that measures the degree of importance given by the relatives to the needs of safety, support, information, comfort and proximity. This study has aimed to detect the degree of importance given by the relatives of the patients in the ICU of the Hospital Dr. Josep Trueta of Girona, using the CCFNI questionnaire. All the relatives of the patients in said unit during the autumn of the year 2003 were interviewed and the needs considered to be most important were compared with the sociodemographic variables of the relatives.
The total sample was made up of the 132 people interviewed. The need considered to be most important was safety with 50.3% followed by need for support with 47.2%. In the results, some differences were observed depending according to age and studies of those surveyed and according to the diagnosis and the origin of the hospitalization. The collected data did not differ with the reviewed bibliography. All the study was very valuable for the health care professionals of the unit and was taken into account to improve the deficiencies of the ICU of the hospital of Girona.

Humanization in healthcare arises from the need for a holistic approach to illness
2018, Medicina IntensivaLa humanización en sanidad surge por la necesidad de acercarnos a una dimensión más holística de la enfermedad. El sufrimiento no solo es exclusivo de los pacientes y las familias, sino que el profesional se encuentra en el centro del proceso de despersonalización. El exceso de tecnificación y la colocación del proceso patológico en ocasiones como único objetivo de actuación, así como la hipertrofia del poder institucional que estamos viviendo en los últimos tiempos, hacen que en ocasiones el propio profesional sanitario sea el primero en demandar un cambio en el abordaje de la dinámica dentro de las instituciones sanitarias. Tras una reflexión inicial, desde el corazón de la medicina más tecnificada, como es la Medicina Intensiva, clásicamente aislada del resto del entorno hospitalario y de las familias, decidimos abordar un proyecto de integración, empatía y acercamiento a los pacientes y familiares de la Unidad de Cuidados Intensivos (UCI) del Hospital Infanta Margarita, en el que se pretendieron implementar herramientas para trabajar en los elementos más importantes de un plan de humanización (las familias, pacientes, profesionales, y nuestra comunidad), potenciando el dar a conocer el trabajo que se realiza en la UCI y que se desarrolló a lo largo de 12 meses, el proyecto: UCI Infanta Margarita, 1 año: 12 meses para 12 compromisos.
Suffering is not only exclusive to patients or their relatives, but also to the health professionals, who feel to be at the center of the depersonalization process. Over-technification and the fact that the disease process is sometimes the only focal point of our activities, together with the ever-increasing influence of institutional power seen in recent times, all cause the health professional to be the first in demanding a change in health institution dynamics. Following initial reflection from one of the most technified medical specialties (Intensive Care Medicine), classically isolated from the rest of the Hospital and from the community, we implemented a project aimed at securing integration and empathy in our approach to patients and their relatives in the Intensive Care Unit (ICU) of Infanta Margarita Hospital. The project was designed to incorporate tools for working on the most important elements of a humanization plan, i.e., the patients, their relatives, the health professionals and the community, attempting to disclose the work done in the ICU over a period of 12 months. This project is referred to as the Project ICU Infanta Margarita: 1 year: 12 months for 12 commitments.
Analysis of needs of the critically ill relatives and critical care professional's opinion

Dos muestras de voluntarios. Una compuesta por el familiar más próximo afectivamente a cada paciente ingresado en la unidad más de 48 h, no primariamente quirúrgico. Otra de profesionales sanitarios con antigüedad superior a 3 meses en la UCI.
Se entregó un cuestionario autoadministrado a cada familiar y otro a cada profesional.
Variables de interés principales: se recogieron datos sociodemográficos. Las variables del cuestionario para familiares fueron: la información recibida, la proximidad al paciente, la seguridad en los cuidados, el apoyo recibido y la comodidad. La empatía y la relación profesional hacia la familia, la política de visitas y el efecto de la familia sobre el paciente fueron las variables del cuestionario para profesionales.
Participó el 59% de los familiares (35/61). El 91,4% comprendió la información recibida, aunque un 49,6% no recibió información sobre cuidados de enfermería. El 82,9% (95,2% eran hijos de pacientes, p < 0,05) mostró conformidad con la política de visitas. La participación profesional fue del 76,3% (61/80). Un 59,3% flexibilizaría la política de visitas y para el 78,3% la familia apoya emocionalmente al paciente sin inestabilizarlo. Un 62,3% informaba habitualmente a la familia, estimando necesaria la formación en habilidades de comunicación un 88%.
La información fue adecuada, resultando insuficiente en cuanto a los cuidados de enfermería. Los profesionales reclamaron formación en habilidades de comunicación.
To describe the needs of the families of patients admitted to the Intensive Care Unit (ICU) and the opinion of ICU professionals on aspects related to the presence of patient relatives in the unit.
A prospective descriptive study was carried out between March and June 2015.
Polyvalent ICU of León University Healthcare Complex (Spain).
Two samples of volunteers were studied: one comprising the relatives emotionally closest to the primarily non-surgical patients admitted to the Unit for over 48 hours, and the other composed of ICU professionals with over three months of experience in the ICU.
One self-administered questionnaire was delivered to each relative and another to each professional.
Main variables of interest: Sociodemographic data were collected. The variables in the questionnaire for relatives comprised the information received, closeness to the patient, safety of care, the support received, and comfort. In turn, the questionnaire for professionals addressed empathy and professional relationship with the family, visiting policy, and the effect of the family upon the patient.
A total of 59% of the relatives (35/61) answered the questionnaire. Of these subjects, 91.4% understood the information received, though 49.6% received no information on nursing care. A total of 82.9% agreed with the visiting policy applied (95.2% were patient offspring; P < .05). Participation on the part of the professionals in turn reached 76.3% (61/80). A total of 59.3% would flexibilize the visiting policy, and 78.3% considered that the family afforded emotional support for the patient, with no destabilizing effect. On the other hand, 62.3% routinely informed the family, and 88% considered training in communication skills to be needed.
Information was adequate, though insufficient in relation to nursing care. The professionals pointed to the need for training in communication skills.
Expectations of relatives of critically ill patients regarding medical information. Qualitative research study

Encontrar claves para mejorar la comunicación y delinear buenas prácticas en materia de información clínica.
Estudio de investigación cualitativa mediante entrevistas semiestructuradas, realizado en una unidad de cuidados intensivos (UCI) polivalente de adultos de un hospital universitario. Población: familiares de pacientes que hubieran estado ingresados y que hubieran sido dados de alta vivos de la UCI. Instrumentos: se realizaron 10 entrevistas considerando como variables de diversificación: tipo de vínculo familiar con los pacientes, edad del paciente, tiempo de estancia en la UCI, origen del enfermo y su localización en el momento de la entrevista.
Del análisis de las 10 entrevistas realizadas, los resultados obtenidos giraron en torno a: la posición subjetiva del familiar en la UCI (la espera angustiosa), lo que representa la UCI para el familiar (vigilancia y control permanente de una situación entre la vida y la muerte), los cuidados percibidos (delegación completa de cuidados) y la información médica (qué y cómo la esperan y qué y cómo la reciben), tanto en la primera información (sinceridad, esperanza, delicadeza) como en las sucesivas.
Existe divergencia entre lo que los familiares esperan y lo que reciben en referencia a la información médica. Conocer las expectativas de las familias nos ayudará a dar una atención de mayor calidad y un trato más humano en nuestras UCI.
To determine and analyse the expectations, needs and experiences of relatives of critically ill patients as regards medical information and the level of their understanding. To find keys for improving communication and to draw up best practices in clinical information.
Qualitative research study through semi-structured interviews carried out in a polyvalent adult intensive care unit (ICU) in a University Hospital. Participants: relatives of patients who were admitted to the ICU and who were discharged alive from the Unit. Ten interviews were performed taking into account diversification variables such as, type of family relationship with patients, patient age, length of ICU stay, origin, and location at the time of the interview.
The results of the analysis of 10 interviews focused on: the subjective position of the family in the ICU (the agonizing wait), what the ICU represents for the family (surveillance and monitoring of a situation between life and death), perceived care (complete delegation of care), and medical information (what and how they expect and what and how they receive it), as much in the first information (sincerity, hope, delicacy) as in the successive.
There is divergence between what families expect and what they get as regards medical information. To know the expectations of the families will help to provide higher quality care and more humane treatment in the ICU.
Satisfaction of relatives of the patients admitted to an intensive care unit: Perceptions of relatives and professionals
2014, Enfermeria IntensivaEstimar la satisfacción de familiares de pacientes críticos con respecto al entorno asistencial, la relación con los profesionales y el régimen de visitas, y compararla con la percepción de los profesionales.
Estudio descriptivo en 2 fases. En la primera fase se realizó encuesta telefónica validada a los 15 días del alta a los familiares de los pacientes. En la segunda fase se entregó el mismo cuestionario modificado autoadministrado para los profesionales.
Se entrevistó a 78 familiares y 44 profesionales. El 95% de los profesionales vs el 67% de los familiares afirmaban no conocer el nombre de los profesionales de enfermería (p < 0,001). Más del 70% de los profesionales y los familiares coincidían en que el protocolo de visitas es correcto y que la información era adecuada al ingreso. El 70% de los profesionales percibieron que la información recibida al ingreso no es comprendida por los familiares, aunque un 97% de los familiares afirmaban sí haberla comprendido. Se observan diferencias estadísticamente significativas en relación con la valoración sobre la localización de la unidad, la sala de espera, la de información y aspectos de la comodidad, siendo siempre los profesionales los que estimaban mayor insatisfacción (p < 0,05).
La percepción de los familiares y los profesionales fue mayoritariamente desigual, por lo que consideramos necesario continuar explorando las diferencias a través de metodologías cualitativas y participativas.
To estimate the satisfaction of relatives of critical care patients with regard to the environment, relationship with professionals and visitation and compare it to the perception of professionals.
A descriptive study in two phases. In the first phase validated telephone survey within 15 days of discharge to the relatives of the patients was performed. In the second phase, the same modified questionnaire was self-administered to the professionals. The same variables were studied in both populations.
We interviewed 78 family members and 44 professionals. 95% of professionals vs 67% of the families claimed not to know the name of the nurses (P < .001). Over 70% of professionals and families agreed that the visit protocol is correct and that the information was adequate income. 70% of professionals felt that the information received at admission is not understood by the family although 97% of households claimed to have understood themselves. Statistically significant differences were observed in relation to the location of the unit assessment, the waiting, the aspects of information and comfort, always being the professionals who felt greater dissatisfaction (P < .05).
The perceptions of relatives and professionals were mostly uneven, making it necessary to continue exploring the differences through qualitative and participatory methodologies.
For an open-door, more comfortable and humane intensive care unit. It is time for change
2014, Medicina IntensivaLa Unidad de Cuidados Intensivos (UCI) es un lugar hermoso donde se regala vida, pero también es un lugar hostil donde los pacientes se enfrentan a una enfermedad terrible en condiciones ambientales muy adversas. Es necesario adaptar tanto el diseño como la organización de la UCI para mejorar la privacidad, el bienestar y la confortabilidad de pacientes y familias, cuidando especialmente sus demandas personales y emocionales. Abrir las puertas de la UCI liberalizando el horario de visitas y mejorar los cuidados dirigidos a la familia es una de las asignaturas pendientes que no debemos retrasar más. Debemos dotar a las UCI de modernos respiradores y equipos de monitorización, pero también debemos invertir en organización, diseño, bienestar ambiental y humanización. Necesitamos rediseñar la práctica clínica para que la atención en la UCI sea más confortable y humana. No se debe aplazar más el cambio ya que es una demanda social y profesional ineludible.
The Intensive Care Unit is a wonderful place where lives are saved, but it is also a very harsh and unpleasant place where critically ill patients face terrible diseases in very adverse environmental conditions. We must change the design of the ICU and its organization; we must improve privacy, welfare and comfort of patients and families, following their personal and emotional demands. To free up the visiting hours and to improve family care are among our most urging matters, which we should delay no further. We must equip the ICUs with modern monitors and respirators but we must also invest in organization, design, environmental comfort and humanization. We need to redesign clinical practise so that ICU care becomes more agreeable and humane. We should put off this change no longer, since it is an imperative social and professional demand.
Benefits of flexible visitation in the intensive care units for the family of critical patients
2012, Enfermeria IntensivaIdentificar, analizar y sintetizar los principales efectos que pueden relacionarse con las visitas flexibles para los familiares del paciente crítico adulto.
Se realizó una revisión de la literatura científica en las bases de datos PubMed, CINAHL, PsycINFO, Cochrane Library y CUIDEN y un análisis temático del contenido de los artículos seleccionados.
Se identificaron un total de 15 publicaciones que respondían al objetivo de la búsqueda. Tras la realización del análisis temático se identificaron 4 temas que incluyen los principales efectos derivados de las políticas de visita flexible para los familiares del paciente crítico: mejora de la satisfacción, reducción de la ansiedad y del estrés, la satisfacción de las propias necesidades familiares y el rol en el cuidado del paciente.
Las políticas de visita flexible, además de ser beneficiosas para los familiares del paciente crítico y el propio paciente, constituyen en sí mismas una necesidad. Se considera necesario favorecer la paulatina participación de la familia en el cuidado y la adquisición del un rol más notorio en el contexto de la visita y en su relación con el paciente.
To identify, analyze and summarize the main effects that may be related to flexible visitation policies for the relatives of critically ill adults.
A review of the literature was conducted in the following databases: PubMed, CINAHL, PsycINFO, Cochrane Library and CUIDEN. Thematic content analysis was used to evaluate selected articles.
Fifteen articles were included in this review. Four main themes emerged from the thematic content analysis. Themes included the main effects of flexible visitation policies for the family of the critical patients such as: improvement of satisfaction, reduction of anxiety and stress, satisfaction regarding their own family needs, and the role of the family in the patient's care.
According to the existing evidence, flexible visitation policies appear to be both beneficial and decisive. It seems to be necessary to favor the participation of the family in the care of the intensive patient as well as the acquisition of a more prominent role the visitation context and in their relationship with the patient.
