Since the failure of CLA is high (nearly 20%),10 excision of the proximal pole could increase this risk. One limitation of our study is the number of times this technique has been carried out, because of the limited number of cases.

One variation with triquetrum excision described by Calendrucio et al. [5] in 2000 makes it easier to position the lunate correctly on the capitate and helps to prevent the peritriquetral arthritis problem described in some studies [2,3,6]. Nevertheless, few studies have analyzed the long-term outcomes of this shortening arthrodesis, which only impacts one joint.

This procedure may be associated with improved functional outcomes [26], although the risk of non-union may be increased [23,26]. As the radiolunate joint may be involved also [5,9], a CT-arthrogram must be obtained routinely before performing four-corner fusion.

The authors concluded that capitolunate arthrodesis represented a good treatment alternative, despite the risk of nonunion.24 Gaston et al23 compared FCA versus capitolunate arthrodesis in conjunction with scaphoid and triquetral excision and reported no nonunions in the latter group, although 5 patients required revision surgery for screw migration.
