ASIA syndrome and endocrine autoimmune disorders

An adjuvant is an immunological or pharmacological substance or group of substances that can be added to a given agent to enhance its effect in terms of efficacy, effectiveness and potency. Different mechanisms have been hypothesized underlying the action of the adjuvant, including boosting immune (innate and adaptive) response: this generally results in sparing the necessary amount of the agent and can potentially reduce the frequency of the needed number of therapeutic interventions. Adjuvants can be commonly found in vaccines, immunization products, mineral oils, cosmetics, silicone breast implants and other therapeutic/medical devices, being usually safe and effective. However, in a fraction of genetically susceptible and predisposed subjects, the administration of adjuvants may lead to the insurgence of serious side-effects, called “autoimmune/inflammatory syndrome by adjuvants” (ASIA) or Shoenfeld's syndrome. The present review is aimed at focusing on the “endocrine pebbles” of the mosaic of autoimmunity and of the ASIA syndrome, collecting together 54 cases of sub-acute thyroiditis, 2 cases of Hashimoto's thyroiditis, 11 cases of primary ovarian failure/primary ovarian insufficiency, 13 cases of autoimmune diabetes type 1, and 1 case of autoimmune adrenal gland insufficiency occurred after exposure to adjuvants.

Introduction

An adjuvant (from the Latin verb “adiuvare”, which has the meaning of “to help, to assist, to aid”) is an immunological or pharmacological substance or group of substances that can be added to a given agent to enhance its effect in terms of efficacy, effectiveness and potency. Different mechanisms have been hypothesized underlying the action of the adjuvant, including boosting immune (innate and adaptive) response: this generally results in sparing the necessary amount of the agent and can potentially reduce the frequency of the needed number of therapeutic interventions [1,2].

Adjuvants can be commonly found in vaccines, immunization products, mineral oils, cosmetics, silicone breast implants and other therapeutic/medical devices, being usually safe and effective [2]. However, in a fraction of genetically susceptible and predisposed subjects, the administration of adjuvants may lead to the insurgence of serious side-effects, due to the activation of autoimmune cascades and pathways, by impairing/disrupting the immunological balance of the host, fostering polyclonal activation of B lymphocytes, by molecular mimicry or other similar etiopathogenetic mechanisms [3].

In order to comprehensively collect together all these phenomena and reactions, ranging from post-vaccination side-effects, and macrophagic myofasciitis syndrome (MMF), to the so-called “functional somatic syndromes”, such as the Gulf war syndrome (GWS), siliconosis, multiple chemical sensitivity, repetition stress injury, the chronic fatigue syndrome (CFS)/myalgic encephalomyelitis (ME), the complex regional pain syndrome (CRPS), the postural orthostatic tachycardia syndrome (POTS) and the sick building syndrome (SBS) [[4], [5], [6], [7], [8], [9], [10], [11], [12], [13], [14], [15], [16]], Shoenfeld and Agmon-Levin in 2011 have coined the umbrella term of “autoimmune/inflammatory syndrome by adjuvants” (ASIA syndrome) [[17], [18], [19], ∗[20]] (Table 1, Table 2).

Vaccine adjuvants

While the story of vaccines dates back to the 18th century, the story of vaccine adjuvants is more recent, dating back to 1926, when the first adjuvants, including aluminum salts, were discovered [21], thanks to pioneers such as Gaston Ramon (1886–1963), Alexander Thomas Glenny (1882–1965), and Jules Freund (1890–1960), a Hungarian born American immunologist, famous for the Freund's adjuvant.

Silicone

Silicone has been utilized for breast gel-filled implants since the fifties-sixties, especially in the field of reconstructive breast surgery. Silicone has always been conceived as a biologically inert material, even though some recent publications have shown its potential role in inducing autoimmune disorders [11,12].

Mineral oils and cosmetic products

Adjuvants can be found also in mineral oils and cosmetic products, especially in South American countries. They belong, indeed, to a category of chemical compounds derived from petroleum, including lubricating base oils and highly refined base oils, able to directly elicit the innate immune system via pattern recognition molecules and to indirectly foster the activation of the adaptive immune system via enhanced antigen presentation [28].

ASIA and the autoimmunity mosaic: the endocrine pebbles

In the present review, we will mainly focus on the endocrine pebbles of the mosaic of autoimmunity and the ASIA syndrome, with the aim of expanding and updating our previously published overview [29].

ASIA and adrenal gland insufficiency

Autoimmune adrenal gland insufficiency (Addison's disease) is a disorder characterized by the production and release of auto-antibodies targeting the steroid-synthesising enzyme 21-hydroxylase, resulting in general fatigue, body weight loss, anorexia, salt craving behavior, low blood pressure and uneven skin pigmentation [30].

Very few cases have been published, linking the occurrence of adrenal gland insufficiency with exposure to adjuvants. For instance, Watad and colleagues reported a case of

ASIA and autoimmune sub-acute thyroiditis

Sub-acute thyroiditis or de Quervain thyroiditis or granulomatous thyroiditis is a generally benign, self-limiting, possibly viral, and inflammatory thyroid disorder that is usually associated with pain affecting the thyroid lodge and systemic symptoms, such as fever, swelling, general fatigue, malaise and sleep disturbances [31]. It represents a rather uncommon cause (less than 5%) of hyperthyroidism.

ASIA and autoimmune Hashimoto's thyroiditis

Hashimoto's thyroiditis or chronic lymphocytic thyroiditis is an autoimmune disorder which gradually results into the destruction of the thyroid gland [45].

Primary ovarian insufficiency/primary ovarian failure

Primary ovarian insufficiency/primary ovarian failure is a complex, multi-factorial disorder, characterized by lymphocytic oophoritis and by auto-antibodies against ovarium antigens of the theca, granulose, corpus luteum, and zona pellucida [47]. Autoimmune type 1 diabetes

Autoimmune type 1 diabetes [58] is a disorder characterized by auto-antibodies targeting pancreatic β cells (anti-islet auto-antibodies). In the scholarly literature, thirteen cases (aged 17.8 ± 17.7 years, range 4–69 years, 7 males, 6 females) of autoimmune type 1 diabetes reported after HBV (11 cases) and HPV vaccine (2 cases) administration were reported by Watad and colleagues [20].

Conclusions and future prospects

Post-adjuvant exposure (vaccine, mineral oils, silicone) phenomena and side-effects appear to be very rare, also because their diagnosis is challenging due to the variety of confounding and masking factors. A systematic review by Jara et al. (61) was able to identify 4479 ASIA cases published in the period between 2011 and 2016, being mainly related to exposure to the HPV and influenza vaccines, silicone, and mineral oil injections.
