Dissemination of effective mental health treatment procedures: Maximizing the return on a significant investment

Abstract

While much is known about the efficacy of evidence-based practices, it is currently less clear how to implement these practices into the broader mental health system. Dissemination and implementation research will play a critical role in addressing this uncertainty. This commentary reviews the most recent and compelling research related to these topics while advocating a broader and more defined perspective of dissemination for future research. Three of the authors' most pressing questions are proposed and explored.

The Feasibility of a Family-Focused CBT Skills Self-Help App

Behavioral intervention technology (BIT) is one avenue for extending the reach of evidence-based pediatric mental health. We developed Anchors Away: Anxiety at Bay, a family-friendly stand-alone mobile health app for children ages 6 to 11 that delivers CBT strategies tailored to mental health struggles typical for youth. This study assessed the acceptability, appropriateness, feasibility, and fidelity following an iterative model of development that provided quantitative and qualitative data. We made changes based on feedback from pilot data and clinical expert stakeholders and conducted a randomized controlled trial (RCT). Findings largely supported the acceptability and feasibility of the app. 71.8% of those randomized to the app show some degree of engagement with the child-side of the app and 47.4% completed all game modules. Most participants (80%) reported that they would purchase the app and found it to be easy to use and helpful in meeting their child’s specific anxiety and emotion regulation challenges.
Supervision in Community Mental Health: Understanding Intensity of EBT Focus

The goal of the present study was to examine clinician, supervisor, and organizational factors that are associated with the intensity of evidence-based treatment (EBT) focus in workplace-based clinical supervision of a specific EBT, Trauma-Focused Cognitive Behavioral Therapy (TF-CBT). Supervisors (n = 56) and clinicians (n = 207) from mental health organizations across Washington State completed online self-report questionnaires. Multilevel modeling (MLM) analyses were used to examine the relative influence of nested clinician and supervisor factors on the intensity of EBT focus in supervision. We found that 33% of the variance in clinician report of EBT supervision intensity clustered at the supervisor level and implementation climate was the only significant factor associated with EBT supervision intensity. While individual clinician and supervisor factors may play a role in EBT coverage in supervision, our results suggest that an implementation climate that supports EBT may be the most critical factor for improving intensity of EBT coverage. Thus, implementation efforts that address the extent to which EBTs are expected, rewarded, and supported within an organization may be needed to support greater coverage of EBT during workplace-based supervision.
A review of the transportability of cognitive therapy for the treatment of PTSD among South African rape survivors

This study aimed to evaluate the transportability of cognitive therapy (CT) for rape survivors with PTSD to South African conditions. Ten local treatment outcome studies investigating the transportability of CT were identified and appraised. The common elements of CT for PTSD including psychoeducation, exposure therapy and cognitive restructuring of trauma-related appraisals were found to be transportable to local contexts. Contextual factors that can complicate treatment delivery were also identified namely exposure to multiple traumatic events, HIV, absence of safety and support in the external environment and language barriers. The augmentations made to an existing evidence-based treatment protocol to address these contextual factors are described.
Use of evidence-based assessment for childhood anxiety disorders in community practice

High-quality assessment is essential to the delivery of effective treatment for childhood anxiety disorders. However, relatively little is known about how frequently child clinicians utilize evidence-based assessment (EBA) techniques in practice, and even less is known about the factors that influence EBA use in such settings. Thus, the current study presents data from a survey of 339 clinicians from a variety of professional backgrounds concerning their use of EBA for childhood anxiety disorders and explores issues preventing EBA implementation. Results indicated infrequent EBA use with clinicians citing practical barriers (i.e., time, access, knowledge, cost) and negative beliefs about EBA techniques (i.e., unhelpful) as issues preventing implementation. Implications for future EBA dissemination and implementation efforts are discussed.
