Differentiation of Raoultella ornithinolytica/planticola and Klebsiella oxytoca clinical isolates by matrix-assisted laser desorption/ionization-time of flight mass spectrometry

Abstract

Ninety-nine clinical isolates previously identified as Klebsiella oxytoca were evaluated using matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS). Eight isolates were identified as Raoultella spp., being 5 Raoultella spp. and 3 K. oxytoca, by 16S rRNA sequencing. These isolates were correctly identified by applying the 10% differential rule for the MALDI-TOF MS score values. This approach might be useful to discriminate Raoultella species from K. oxytoca.

    Hence, it was initially termed Klebsiella planticola, however, differences in 16S rRNA and rpoB genes lead to a definition of the new genus Raoultella [2]. This genus includes Raoultella planticola, ornithinolytica, terrigena and electrica [3–6].     R. ornithinolytica previously was thought to rarely cause infection in humans; however, recently it has been reported in the literature as causing many different types of infections including bacteremia, cholangitis, urinary tract infection, pneumonia, skin infections, osteomyelitis, meningitis, cerebral abscess, mediastinitis, pericarditis, conjunctivitis, otitis, among other infections [2,3]. The increase in reporting of R. ornithinolytica is likely due to implementation of new identification techniques in the laboratory such as the matrix-assisted laser desorption/ionization-time of flight mass spectrometer (MALDI-TOF MS) as older techniques did not always differentiate between Klebsiella spp and Raoultella spp [2,4]. In the most comprehensive study to date, Seng et al. reported 112 cases of R. ornithinolytica infections over an eleven year period which included both community and hospital-acquired infections from different sources [2].

    The speed and the identification rate of classic and rare bacterial species involved in human infection have increased significantly since the arrival of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) in the authors’ centres.10 Recently, MALDI-TOF MS has been used successfully in the differentiation of R. ornithinolytica from Klebsiella oxytoca.11,12 The aim of this study was to describe the clinical and biological characteristics and clinical outcomes of human infections caused by R. ornithinolytica managed in the authors’ centres during the period before and after the introduction of MALDI-TOF MS. This study was presented as a poster at the 25th European Congress of Clinical Microbiology and Infectious Diseases, Copenhagen, Denmark (ECCMID 2015).
