Zusammenfassung

Einleitung

Aufgrund der demographischen Entwicklung ist mit einer deutlichen Zunahme von Sprunggelenksfrakturen beim älteren Patienten zu rechnen.

Material und Methode

Komplexe Frakturmuster, osteoporotische Knochenqualität und Komorbiditäten machen die Therapie dieser Verletzungen zu einer speziellen Herausforderung. Auch beim älteren Patienten steht die Wiederherstellung der Funktion an oberster Stelle des therapeutischen Regimes.

Ergebnisse

Die konservative Therapie wird bei stabilen Frakturformen sowie instabilen Frakturen mit Kontraindikationen zur operativen Therapie empfohlen. Die operative Therapie sollte weichteiladaptiert und durch spezielle operative Techniken und Implantate individuell erfolgen. Hierbei hat die initiale Transfixation mittels Fixateur externe einen hohen Stellenwert.

Diskussion

Die operativen Techniken reichen von dorsaler Plattenpositionierung über K-Draht-Techniken, Hakenplatten, Fibulanägeln bis hin zu winkelstabilen Plattensystemen, die gute postoperative Ergebnisse zeigen. Eine primäre Versteifung mittels retrograder Nagelarthrodese ist nur als Salvage Verfahren Ausnahmefällen vorbehalten.

Background

Because of demographic changes over the next years, there will be an increase of osteoporotic ankle fractures in the elderly.

Material and methods

Complex fracture pattern, osteoporotic bone quality and comorbidities of the patients pose a challenge in therapy and outcome. Functional rehabilitation is the primary goal of treatment regardless of patient age.

Results

The goal of operative treatment is reconstruction of the function and exact anatomy of the ankle joint with respect to the soft tissue trauma.

Discussion

Preoperative CT scanning is recommended for exact diagnosis and preoperative planning; surgical options for stabilization are locking plates, dorsal plate positioning, additive K-wire fixation, intramedullary devices and second stage reconstruction with initial use of external fixation. Only in exceptional cases, primary arthrodesis using a retrograde intramedullary nail should be considered as a salvage procedure.

Einleitung

In Anbetracht der aktuellen und zu erwartenden demographischen Entwicklung zeigt sich eine steigende Inzidenz von Frakturen des oberen Sprunggelenkes bei älteren Patienten [5], [13].

Die Sprunggelenksfraktur an sich ist keine typische osteoporotische Verletzung, bimalleoläre und trimalleoläre Frakturen zeigen jedoch Kriterien einer osteoporotischen Fraktur [9]. Risikofaktoren für die Entstehung einer Sprunggelenksfraktur im Alter sind das weibliche Geschlecht, höheres Körpergewicht und Diabetes

Fazit

Aufgrund der demographischen Entwicklung ist bereits eine Zunahme der Sprunggelenksfrakturen beim älteren Patienten zu verzeichnen und ist mit einer weiteren Zunahme zur rechnen [13].

Die Behandlung dieser Verletzungen stellt aufgrund der oftmals kompromittierten Weichteilverhältnisse, der schlechten Knochenqualität, vorbestehender Deformitäten sowie degenerativer Veränderungen, der Notwendigkeit einer Frühmobilisation und Vermeidung einer längeren Immobilisation eine Herausforderung dar.
