Integrated Care: Enhancing the Role of the Primary Health Care Professional in Preventing Functional Decline: A Systematic Review
2017, Journal of the American Medical Directors AssociationCitation Excerpt :In order to identify frail older persons at risk of negative outcomes, general practitioners must be provided with an easy and quick screening tool for detecting frailty without special effort. The GFST (Table 2)26,27 has been developed and implemented in primary care with the collaboration of the Department of Family Medicine of the University of Toulouse. The GFST is designed to be administered to persons aged ≥65 years at risk for frailty.
Risk factors for functional decline in a population aged 75 years and older without total dependence: A one-year follow-up
2016, Archives of Gerontology and GeriatricsCitation Excerpt :Primary care is the most appropriate setting to detect and take care of frailty. However, the identification of the frailty syndrome is still too complex to be considered clinically friendly (Lacas & Rockwood, 2012; Rouge Bugat, Cestac, Oustric, Vellas, & Nourhashemi, 2012). Few measures have been validated in a primary healthcare setting, and not many studies recruit patients directly in primary care consultations (Pialoux, Goyard, & Lesourd, 2012).
