Case report

A 35-year-old man without preexisting diseases was admitted after a 3-day history of fever >39°C, neck pain, left-sided swelling of the neck, and dysphagia. Physical examination revealed a left-sided peritonsillar abscess. White blood cell count was 31.8 g/L and C-reactive protein (CRP) was 55 mg/dL. Discussion

A cervical necrotizing fasciitis has specific characteristics distinguishing it from other localizations.7 Although in most regions the surgical treatment can be more aggressive, the face and neck region harbors a multitude of vital structures in a relatively small area, requiring a careful necrosectomy with preservation of as much muscle, nerves, and vessels as possible. The initial surgical treatment in this patient might be considered as too cautious and the resulting mediastinitis developed
