Recently, a good result was reproduced in patients with MRI-negative idiopathic TN with the method proposed by Ishikawa et al.11,14 Because reports regarding arachnolysis for MRI-negative TN are rare, it is likely that an alternative procedure was performed if MRI did not show clear neurovascular compression. We came across a patient who experienced a complete cure of TN that lasted for 20 years after only intradural arachnolysis of the trigeminal nerve, because there were no offending vessels during MVD 5 years ago (Figure 1A).
