Zusammenfassung

Die Rettung und medizinische Versorgung von Ertrinkungsunfallopfern ist ein seltenes Ereignis und bedarf gerade deshalb der besonderen Beachtung. Die Rettung kann technisch schwierig sein und nur ein eingespieltes Team kann dies professionell und ohne Zeitverlust durchführen. Die Eigensicherung steht hierbei an erster Stelle. Die Zusammenarbeit von technischem und medizinischem Rettungspersonal ist außerordentlich wichtig und muss der Inhalt von Übungen sein. Unterkühlte Personen sollten schonend und horizontal gerettet werden und vor weiterer Auskühlung geschützt werden. Unterkühlte Patienten ohne eigenen Kreislauf sollten ohne Zeitverzögerung unter kontinuierlicher Reanimation in eine geeignete Zielklinik gebracht werden, die über eine Möglichkeit der extrakorporalen Wiedererwärmung verfügt. Eine rechtzeitige Anmeldung sowie die Auswahl des schnellsten Transportmittels sind hier unerlässlich, um keine Zeit zu verlieren. Wie eindrucksvolle Einzelfallbeschreibungen zeigen, ist dieses Verfahren auch nach langer Submersionszeit erfolgversprechend.

Abstract

Rescue and medical care of people in a drowning accident is a rather rare incident which still needs special attention. The rescue process can be technically challenging and only a well experienced team will be able to act professionally without any time loss. At a first step all team members have to protect themselves. Especially close collaboration of technical and medical rescue teams are of high significance and should be part of future exercises. Hypothermic persons should be protected from further cooling and gently rescued in a horizontally way. If a patient has no circulation continued cardiopulmonary resuscitation and immediate transport as soon as possible to the nearest hospital with an extracorporeal rewarming device is recommended. To avoid any time loss it is essential that the respective hospital is informed immediately and the fastest mode of transport selected. Astonishing cases show that this procedure is very promising even after long time submersion.

Kernaussagen

  • Eine Unterscheidung in Süßwasser– oder Salzwasserertrinken ist nicht sinnvoll und hat keine Relevanz für die präklinische und innerklinische Versorgung der Patienten.

  • Begleitverletzungen oder Erkrankungen, die zum Ertrinkungsunfall geführt haben, dürfen nicht vernachlässigt werden.

  • Die Rettung von unterkühlten Ertrinkungsunfallopfern sollte möglichst horizontal und immobilisiert erfolgen, um ein Einschwemmen von peripher kaltem Blut nach zentral zu vermeiden.

  • Hypoxie ist das führende klinische Symptom und muss ohne Zeitverlust nach geltenden Standards behandelt werden.

  • Der Notarzt muss sich frühzeitig um die weitere klinische Versorgung des Patienten kümmern und ggf. ein Krankenhaus mit der Möglichkeit einer extrakorporalen Erwärmung zur weiteren Therapie anfahren (ggf. luftgebunden).

  • Ein Abbruch von Reanimationsmaßnahmen bei einem unterkühlten Patienten ist präklinisch nicht sinnvoll.
