Zusammenfassung

Die symptomatische Therapie der Myasthenia gravis mit Hemmstoffen der Acetylcholinesterase ist neben der immunsuppressiven Behandlung von entscheidender Bedeutung. Hierfür stehen mehrere Substanzen – insbesondere Pyridostigmin – zur Verfügung. Cholinesterase-Hemmstoffe können auch bei einigen anderen Erkrankungen mit gestörter Funktion der neuromuskulären Transmission hilfreich sein. Ergänzend besteht eine Therapieoption des Lambert-Eaton-Syndroms mit 3,4-Diaminopyridin bzw. Amifampridine. In dieser Übersicht wird ausführlich auf die Pharmakologie sowie Einzelheiten der Therapie mit diesen Medikamenten eingegangen. Darüber hinaus werden einige Situationen bei Myasthenia gravis (u. a. ältere Patienten, juvenile myasthene Syndrome, Narkose, Schwangerschaft) dargestellt, die die Behandlung dieser Erkrankung erschweren können.

Abstract

The symptomatic treatment of myasthenia gravis using inhibitors of acetylcholine esterase is – besides immunosuppression – of outstanding importance. Several cholinesterase inhibitors are available, especially pyridostigmine. These compounds are useful in the treatment of numerous other disorders of neuromuscular transmission, too. Besides these drugs there is an important treatment option for patients with the Lambert-Eaton myasthenic syndrome, 3,4-diaminopyridine or amifampridine. In this review we discuss the pharmacology as well as important details of treatment with cholinesterase inhibitors. Moreover, numerous situations within the course of myasthenia gravis, e. g., older patients, juvenile myasthenic syndromes, anesthesia, and pregnancy which could complicate treatment of myasthenia are presented in the context of cholinesterase inhibitor treatment.
