Zusammenfassung

Hintergrund Das Ziel dieser prospektiven Pilotstudie ist es, unsere ersten Ergebnisse der lokalen Anwendung von Eigenserum (=autologem Blutserum) bei therapieresistenten Hornhautepitheldefekten darzustellen. Insbesondere sollen Effektivität, Praktikabilität und Sicherheit dieser neuartigen Behandlungsmodalität evaluiert werden.

Methoden Siebzig Augen von 67 Patienten im Alter zwischen 18 und 92 Jahren (59 ± 18 Jahre) wurden im Zeitraum von November 1999 bis Juni 2001 konsekutiv mit Eigenserum-Tropfen therapiert. Wir untersuchten Lokalisation, Größe und Dauer des Defektes, die Zeitdauer bis zum vollständigen Epithelschluss und die Rezidivrate in Bezug zur klinischen Diagnose. Nach Zentrifugieren von frisch abgenommenem Eigenblut wurde das Serum in sterilen Tropffläschchen bei Kühlschranktemperatur (+ 4 °C) aufbewahrt und tagsüber unverdünnt stündlich getropft. Die zusätzliche lokale und systemische Medikation war auf die Grunderkrankung des Epitheldefektes abgestimmt.

Ergebnisse Bei 45 Augen bestand ein Epitheldefekt nach perforierender Keratoplastik, bei 25 Augen war der Epitheldefekt nicht mit einem Hornhauttransplantat assoziiert. Der Epitheldefekt war im Mittel 4,2 ± 2,4 mm lang und 3,0 ± 1,8 mm breit. Alle Patienten hatten zuvor maximale lokale Therapie (inklusive Hyaluronsäure) für 13 ± 11 Tage bekommen. Die Eigenserumbehandlung erfolgte zwischen 4 und 45 (16 ± 11) Tage. Bei 57 von 70 Augen (81 %) war der komplette Epithelschluss nach 3 bis 45 (im Mittel 15 ± 12) Tagen zu erzielen. Achtundvierzig dieser 57 Augen (84 %) blieben während einer Nachbeobachtungszeit von 12 ± 4 Monaten rezidivfrei. Bei 9 Augen (16 %) trat im ersten oder zweiten Monat nach Beendigung der Eigenserumtherapie erneut ein Epitheldefekt auf, so dass für einen Erfolg 6 × eine zweite Eigenserumanwendung, 2 × eine Amnionmembran-Transplantation und 1 × eine Re-Keratoplastik nötig wurden. Bei 13 Augen (19 %) wurde zwischen 11 und 50 Tagen die Therapie erfolglos abgebrochen und eine Amnionmembran-Transplantation (10 ×) oder Re-Keratoplastik (3 ×) vorgenommen.

Schlussfolgerung Die lokale Eigenserumanwendung stellt bei therapieresistenten Epitheldefekten der Kornea eine praktikable und effiziente Therapie dar. Insbesondere bei Oberflächen-Insuffizienz nach komplizierter perforierender Keratoplastik kann so die Prognose verbessert werden und invasivere Verfahren, wie Botulinumtoxin-Applikation zur protektiven temporären Ptosis, Amnionmembran-Transplantation oder eine Re-Keratoplastik, meist vermieden werden. Eindeutige Determinanten für den Erfolg dieser Therapie konnten bisher noch nicht identifiziert werden. Allerdings scheinen uns Augen mit zusätzlichen tiefen stromalen Defekten keine guten Kandidaten zu sein.

Purpose The purpose of this prospective clinical pilot study was to evaluate the efficiency, practicability and safety of topical autologous serum for therapy resistant corneal epithelial defects.

Methods Between November 1999 and June 2001 autologous serum therapy was applied in 70 eyes of 67 consecutive patients (age 18 to 92 [average 59 ± 18] years) with corneal epithelial defects refractory to standard clinical treatment at the Department of Ophthalmology University Erlangen-Nürnberg. We evaluated localization, size of the defect, time until epithelial wound closure and the incidence of recurrences in relation to the clinical diagnosis. The blood was obtained by venipuncture, centrifuged and the non-diluted serum was filled in sterile bottles, kept in the + 4 oC refrigerator and hourly applied during daytime. Simultaneous systemic and topical medication depended on the underlying disease.

Results In 45 eyes corneal epithelial defects appeared after penetrating keratoplasty but in 25 eyes corneal epithelial defects were not related to a corneal transplant. The epithelial defect had a medium length of 4.2 ± 2.4 mm and a width of 3.0 ± 1.8 mm. Previously, the patients had been treated with maximal topical therapy (including hyaluronic acid) for 13 ± 11 days. Treatment with autologous serum lasted from 4 to 45 (mean 16 ± 11) days. A complete corneal re-epithelialization was achieved in 57 of 70 eyes (81 %) after 3 to 45 (mean 15 ± 12) days. Forty-eight of these 57 eyes (84 %) had no recurrence during a follow-up of 12 ± 4 months. In 9 eyes (16 %) a recurrent epithelial defect occurred one or two months after initial closure. These eyes were treated sucessfully with a second autologous serum therapy (6 ×), amniotic membrane transplantation (2 ×) or repeat keratoplasty (1 ×). Thirteen eyes (19 %) without primary success of autologous serum applied for 11 to 50 days were finally treated successfully with amniotic membrane transplantation (10 ×) or repeat keratoplasty (3 ×).

Conclusion In persistent corneal epithelium defects, autologous serum therapy can be considered as an effective and practicable therapy without adverse reactions. Especially in eyes after complicated penetrating keratoplasty the prognosis may be improved and more invasive treatment modalities such as botulinum toxin injection, amniotic membrane transplantation or (repeat) penetrating keratoplasty may be avoided. Definitive determinants for the success of this novel therapy have not been identified, yet. However, eyes with accompanying deep stromal defects do not seem to be good candidates.
