Subhyaloidale makuläre Blutung als mögliche Komplikation einer atypischen Zentralvenenthrombose

Zusammenfassung

Hintergrund: Subhyaloidale makuläre Blutungen sind eine seltenere Komplikation bei Zentralvenenthrombosen und Venenastthrombosen, bei vaskulären Malformationen, im Rahmen eines M. Terson, Traumata oder nach Valsalvamanövern. Wir berichten über eine junge Patientin mit völlig unauffälliger Allgemeinanamnese, die eine große subhyaloidale makuläre Blutung im Rahmen einer atypischen Zentralvenenthrombose zeigte. Anamnese und Befund: Eine 25-jährige, ansonsten gesunde Patientin stellte sich wegen einer akut aufgetretenen starken Visusminderung des rechten Auges vor. Als Grund dafür fanden wir eine ausgeprägte makuläre subhyaloidale Blutung sowie gestaute, tortuöse, retinale Venen und einige flamm- und fleckförmige präretinale Blutungen im Bereich dieser Gefäße. Weiterhin war ein leichtes Ödem entlang einer zilioretinalen Arteriole inferior der Makula nachweisbar. Außer einem seltenen Nikotingenuss war die Patientin nach eigenen Angaben völlig gesund und anamnestisch konnte ein Valsalvamanöver nicht ausgeschlossen werden. Schlussfolgerung: Eine atypische Zentralvenenthrombose kann in seltenen Fällen einhergehen mit einer subhyaloidalen makulären Blutung. Differenzialdiagnostisch kommt bei unserer Patientin auch ein stattgefundenes Valsalvamanöver in Betracht. Dieses könnte zu der subhyaloidalen und den präretinalen Blutungen und durch einen temporär erhöhten venösen Druck zu einem Zentralvenenverschluss geführt haben. Sekundär ist es unter Umständen zu dem Verschluss einer zilioretinalen paramakulären Arterie gekommen.

Abstract

Background: Subhyaloid macular bleeding is a rather rare complication caused by central or branch retinal vein occlusions, by vascular malformations, Terson's syndrome, trauma or valsalva's manoeuver. We report on a young and otherwise healthy patient with a large subhyaloid macular hemorrhage associated with an atypical central vein occlusion. History and signs: A 25-year old female was evaluated for an acute and dramatic drop of visual acuity of the right eye. The ophthalmological findings included an extensive subhyoidal macular bleeding, congested, tortuous retinal veins and some flame-shaped preretinal haemorrhages along a couple of veins. Furthermore, a slight edema in the area of an inferiorly of the macular region localized cilioretinal artery was visible. The general history of the patient was unremarkable with the exception of smoking. Anamnestically, Valsalva's manoeuver could not be excluded by the patient. Conclusion: Atypical central vein occlusions can be the rare cause of a subhyaloid macular hemorrhage. A Valsalva's manoeuver could have caused in our patient the subhyaloid macular and the preretinal bleedings. The temporarily raised venous pressure could have been responsible for the central vein occlusion, secondarily causing the occlusion of a cilioretinal paramacular artery.
