Zusammenfassung

Einleitung: Das maligne Mesotheliom der Tunica vaginalis testis ist ein sehr seltener hochmaligner Tumor des Hodens bzw. der Hodenhüllen. Die Prävalenz für Mesotheliome liegt bei 1 : 1 000 000, wovon wiederum nur ca. 1 % von der Tunica vaginalis testis ausgehen. Die 5-Jahres-Überlebensrate liegt bei < 5 %. In der Weltliteratur sind bisher nur ca. 80 Fälle beschrieben, bei denen in 41 % anamnestisch eine vorherige langjährige Asbestexposition eruierbar war. Kasuistik: In unserem Fall handelte es sich um einen 76-jährigen Patienten mit einer atypischen Hydrozele testis. Intraoperativ zeigten sich maximal verdickte Hodenhüllen und ein kristalliner Rasen, der an die Tunica vaginalis testis angelagert war. Schlussfolgerung: Im Zusammenspiel mit dem präoperativen Ultraschallbefund im Sinne eines schneegestöberartigen Binnenechos, der trüben Hydrozelenflüssigkeit mit sichtbaren Kristallpartikeln und den massiv verdickten weißlichen Hodenhüllen, muss man an ein Mesotheliom der Hodenhüllen denken. Eine intraoperative Schnellschnittuntersuchung und eine hoch inguinale Absetzung mit Semikastration ist dann die Operationsmethode der Wahl.

Abstract

Introduction: Malignant mesothelioma of the tunica vaginalis is a very rare malignant tumour. The prevalence of mesothelioma is about 1 : 1 000 000: Only 1 % have their origin in the tunica vaginalis testis with a 5-year survival of less than 5 %. About 80 cases have been reported in the literature. In 41 % of these cases exposure to asbestos for many years was determines. Case Report: We report on a 76-year-old patient who presented with an atypical hydrocele testis. Intraoperatively, thickened testicular walls and an crystalline lawn adjacent to the tunica vaginalis testis were found. Conclusions: Ultrasonography showing snow flurries together with cloudy hydrocele fluid and visible crystal particles and thickened testicular walls may help to identify a mesothelioma of the testicular walls. Intraoperative frozen section and a high inguinal orchiectomy is the operative method of choice.
