Zusammenfassung

Ungestaltete und gestaltete, elementare Ohrgeräusche oder Akoasmen werden auch bei psychiatrischen Erkrankungen als Symptome unterschiedlichen Ursprunges und individueller Erlebnisverarbeitung gesehen. Im Rahmen eines mehrdimensionalen Bedingungsgefüges von akustischen Wahrnehmungsstörungen entscheiden Läsion und Kompensationsvermögen des Sinnessystems, primäre Persönlichkeitsstruktur, psychodynamische Konfliktbewältigungsstrategie und vor allem die konstitutionell vorgegebene oder erworbene, zentralnervöse biologische Struktur darüber, was aus der objektiven Funktionsminderung des Hörens in der Erlebnisrepräsentanz subjektiv wird und klinisch als Ohrgeräusch imponiert. Neben psychopathologischen und neuropsychophysiologischen Aspekten wird kurz die Klinik von Ohrgeräuschen gestreift sowie anhand einer Kasuistik die akustische Halluzinose der Schwerhörigen dargestellt und in ihrer Pathogenese diskutiert.

Summary

Unstructured and structured, elementary tinnitus or achoasms are regarded as symptoms of various origins and individual processing of experience even in psychiatric diseases. In the context of a multidimensional causal structure for disturbances of acustic perception, the lesion and compensatory capacity of the sensory system, primary personality structure, strategy for psychodynamic conflict resolution and above all the constitutionally determined or acquired biological structure of the central nervous system decide on what becomes subjective experience from the objective reduction of auditory function and what is observed clinically as tinnitus. Besides psychopathological and neuropsychophysiological aspects, the clinics of tinnitus is touched on and acustic hallucinosis of the hard of hearing is illustrated by reference to a case history and its pathogenesis is discussed.
