However, studies have demonstrated that the forearm BMD correlates well with central BMD measurements (such as lumbar and total hip) [23–25], and this evaluation has been used in previous studies of BMD in DMPA users [25–27]. Our study has some limitations such as not taking into account dietary calcium intake, women's previous history of contraceptive use, history of family osteoporosis and other risk factors for bone loss.

Despite the evidence that use of DMPA is associated with a reduction on BMD at different sites of the skeleton including measurement at forearm [15–21], there are scarce data regarding the skeletal health impact of this contraceptive method on long-term users and also the impact of DMPA use on fractures and fracture risk [22–27]. Consequently, the objective of this study was to assess forearm BMD of a cohort of women who have been using DMPA as a contraceptive method for as long as 15 years uninterruptedly and compared them with never users of the method.

Although, PBMD is not used to diagnose osteoporosis (6) or to monitor the therapeutic efficacy of antiresorptive drugs (7), its ability to predict fractures has been proven in epidemiological studies (8). Despite limitations in clinical applications, these peripheral devices will remain available for clinicians, as central DXA facility is available only in few major cities in Sri Lanka.
