The C-shaped canal configuration is a developmental anomaly that may not be identified on preoperative radiographs. Clinical findings include normal anatomy of the pulp floor, separate canal orifices, and continuous sensitivity and hemorrhage.

In three cases, the preoperative radiographs and internal anatomies of the pulp chamber indicated normal anatomy of mandibular second molars. As treatment progressed, it became apparent that C-shaped roots and canals were present.
