Zusammenfassung
Das Nationale Referenzzentrum (NRZ) für Meningokokken ist in Zusammenarbeit mit dem Robert-Koch-Institut (RKI) verantwortlich für die Laborsurveillance der Meningokokkeninfektion in Deutschland. Das NRZ Meningokokken hat in den vergangenen Jahren im Konsens mit europäischen Referenzlaboratorien und der European Monitoring Group on Meningococci (EMGM) die Typisierung der Meningokokken auf DNA-Sequenz-basierte Verfahren umgestellt. Typisierungsdaten werden für Computer-gestützte Clusteranalysen verwendet und durch das geografische Informationssystem EpiScanGIS visualisiert. Die Daten des NRZ Meningokokken werden mit den Meldedaten des RKI abgeglichen, so dass Deutschland über das RKI einen qualitativ hochwertigen Datensatz an das European Centre for Disease Control and Prevention (ECDC) übermitteln kann. Die Abdeckung der in primären Laboratorien erfassten Infektionen durch das NRZ Meningokokken ist dank des Engagements der Primärlaboratorien hervorragend. In diesem Artikel werden die Diagnostik der Meningokokkeninfektion, die Antibiotikaresistenzbestimmung und die Feintypisierung aus Sicht des NRZ Meningokokken dargestellt. Fragen der Laborsicherheit werden zusätzlich erörtert.
Abstract
The German Reference Laboratory for meningococci is responsible for laboratory surveillance of meningococcal disease in Germany and collaborates closely with the Robert Koch Institute. Bacterial typing beyond serogrouping is conducted exclusively using DNA sequence-based techniques, in consensus with the European Monitoring Group on Meningococci. Typing data are used in computer-assisted detection of spatio-temporal clusters of disease. They are visualized using the EpiScanGIS geographic information system. Data from the reference laboratory are matched to national notification data at the Robert Koch Institute to provide a comprehensive data set of high quality for submission to the TESSy database of the European Centre for Disease Control and Prevention. The estimated coverage of notified cases is satisfactory because of the efforts of laboratories that submit data. In the present review, diagnosis of Neisseria meningitidis, antimicrobial resistance testing, and fine typing are discussed from the viewpoint of the reference laboratory, including issues related to laboratory safety management.
