Lokale und systemische Leichtkettenamyloidosen bei Patienten mit rheumatologischen Erkrankungen

Zusammenfassung

Hintergrund

Bei Patienten mit chronisch-entzündlichen rheumatologischen Erkrankungen können verschiedene Arten von Amyloidoseerkrankungen auftreten. Wenn eine Amyloidose vermutet wird, muss zwischen lokalen und systemischen Amyloidablagerungen sowie AL (Leichtketten), AA (Amyloid A) und anderen Amyloidoseformen unterschieden werden.

Ziel

Ziel ist die Charakterisierung der lokalen und systemischen AL-Amyloidose bei rheumatologischen Erkrankungen, das Aufzeigen diagnostischer Algorithmen und prognostischer Faktoren sowie die Diskussion von Therapieoptionen.

Material und Methoden

Es erfolgen die Darstellung der Amyloidosekohorte am Amyloidose-Zentrum Heidelberg, der Vergleich mit anderen Amyloidosekohorten sowie die Diskussion der Therapieoptionen.

Ergebnisse

Eine monoklonale Gammopathie kann bei verschiedenen rheumatologischen Erkrankungen auftreten. Beim Sjögren-Syndrom wurden knotige kutane Ablagerungen von lokalem AL-Amyloid beschrieben. Das Auftreten einer systemischen AL-Amyloidose wurde bei rheumatologischen Erkrankungen bisher allerdings nur in wenigen Einzelfällen berichtet.

Schlussfolgerung

Bei Patienten mit rheumatologischen Erkrankungen muss, neben der AA-Amyloidose auch an die Entwicklung einer meist lokalen AL-Amyloidose gedacht werden. Bei Hinweisen auf eine systemische Amyloidose ist die frühzeitige Diagnosestellung notwendig, damit die Funktion der betroffenen Organe erhalten werden kann. Um eine zielgerichtete Therapie beginnen zu können, muss sicher zwischen einer AA- und AL-Amyloidose differenziert werden.
