Lymphome und lymphatische Leukämien im Knochenmark

Zusammenfassung

Eine Knochenmarkstanze mit der Frage einer Lymphommanifestation wird häufig im Rahmen von Stagingmaßnahmen durchgeführt, d. h. mit der Frage nach einem Befall des Knochenmarks durch einen bereits (extramedullär) diagnostizierten Tumor. Nicht selten führen aber auch klinisch noch unerklärte Blutbildveränderungen oder sonstige Befunde wie ein M-Gradient zur bioptischen Abklärung des Knochenmarks, in deren Verlauf dann die Erstdiagnose eines Lymphoms gestellt wird. In jedem Fall erfordert die Diagnostik maligner Lymphome und lymphatischer Leukämien im Knochenmark in ganz besonderer Weise eine Integration von klinischen Daten, Zytomorphologie, Topographie des Infiltrats sowie immunhistochemischen und ggf. auch molekularpathologischen Verfahren. Dies insbesondere, wenn es um den Nachweis oder Ausschluss konventionell-morphologisch okkulter Infiltrate (z. B. Haarzell-Leukämie, hepatosplenisches T-Zell-Lymphom, Plasmazellinfiltrate) und deren genaue Quantifizierung geht.
